To recruit and train highly promising Kenyan scholars (2 PhD, 4 Master’s students). To provide seed funding and ongoing mentorship for graduates to initiate new research projects in Kenya.

Aim# 2. To strengthen the capacity of Kenyan faculty and researchers to conduct research in HIV and cervical cancer screening, prevention and treatment by offering medium term training. This will encompass clinical,

epidemiologic, behavioral, implementation, laboratory capacity and basic science research. We also aim to increase the number of Kenyan women scientists.

AIM# 3. To establish annual symposiums/workshops for research on HIV and cervical cancer based in Kenya, for dissemination of research findings, professional networking and enhancing research projects. To host one-week

2. AMPATH-Oncology Institute: HPV and Cervical Cancer in Kenyan Women with HIV/AIDS

My Role: Principal Investigator (PI) Funding Agency: NIH Type: U54 Period: 19/9/2014- 31/8/2019 Cost: $728,206 (Project Total) This project aims to assess the results of cryotherapy and LEEP among VIA positive HIV- and HIV+ women in western Kenya. Modifiable risk factors that lead to treatment failure and HPV types (persistence and alterations) also will be assessed.

3. Novel concepts for cervical cancer screening in two rural African regions Gondar, Amhara region, Ethiopia and Eldoret, Kenya)

My Role: Co-Investigator Funding agency: Germany Ministry of Higher Education (BMBF-Programme) Cost: $ 238,429 Primary HPV testing is the most effective risk screening tool with a very high sensitivity, but less high specificity due to spontaneous clearance of the infection especially in young women. An increasing amount of studies underline that CINtec®PLUS is the most accurate disease marker having high sensitivity and specificity to detect the pre-cancer stage. Our aim is to compare the performance of VIA with HPV and CINtec®PLUS. To a cohort of about 800 eligible women aged 18 years or above attending one of two clinics in the Uasin-Gishu County of Kenya we will offer concurrent screening with HPV DNA test, CINtec®PLUS and VIA conducted by a trained nurse.

4. Home Based Self-Collected Vaginal Swabs for HPV Testing.

Cost: $150,000. Through this project we aim to provide knowledge and cervical cancer screening to all women in the Chulaimbo region. The FLTR program that is already in place within AMPATH in Western Kenya represents an ideal opportunity to determine if individualized education and encouragement to go for screening at the household level will impact a population with an extremely high risk of developing and dying from cervix cancer.

5. Leeping Ahead: See and Leep training for primary care providers in Kenya.

Aim 2 will include 400 HIV-infected women with abnormal VIA needing LEEP.

6. Taking a LEEP! Implementing a “See and LEEP” strategy for women in Western Kenya with positive cervical cancer screening.

My Role: Principal Investigator. Funding Agency: Grand Challenges Canada Type: Period: Cost: CAD 94,380 We propose a “See and LEEP” strategy in rural Kenya to provide a point-of-need service for women with a positive cancer screen. LEEP is highly effective at treating pre-malignant disease, has low morbidity and can be used in a low-resource setting.

Association of Obstetricians and Gynaecologists of Uganda, Uganda – The Society of Obstetricians and Gynaecologists of Canada, Canada Summary/ Aim: The proposed intervention, Advances in Labour and Reducing Mortality (ALARM) – International Programme (AIP) training, will help the Ministry of Health implement the Kenya Essential Package for Health (KEPH) in western Kenya. Final report has been written and submitted to the funding agency through the British Council.

4. WHO Pilot Study to Assess the Feasibility of Medicines in Pregnancy

Registry. My Role: Co- Principal Investigator This protocol developed an approach to understanding the potential for

teratogenicity of medicines used on a wide-scale in resource-limited settings. ACTs for malaria served as the initial focus. A key objective was to collect high-quality evidence that would be sufficient to reinforce or change current antimalarial drug policy recommended by WHO regarding use of the artemisinins in pregnancy.

5. Ampath Cervical Cancer Screening Program (ACCSP)

Cost: $100,000 Cervical Cancer Screening Program includes the training of ancillary health care providers in cervical screening, as well as machines for the performance of colposcopy and cryotherapy for the treatment of dysplastic appearing lesions. We employ a see and treat strategy. Clients are screened by use of Visual inspection with Acetic acid (VIA) and those found to have abnormal epithelium (Acetowhite) are treated with cryotherapy. This is a cost-effective approach in resource-constrained setting like Kenya.

6. Screening for Cervical Cancer in HIV-Positive Kenyan Women: The Role of

Human Papillomavirus Typing.

Cost:

This was a funded study that aimed to describe the prevalence of HPV

genotypes in HIV infected women who are candidates for cryotherapy and

those with biopsy-confirmed severe dysplasia and invasive cervical cancer in

Eldoret, Kenya. Manuscript is under development.
