Tongue guards were missing and work rests were improperly adjusted, exposing employees to the hazard of shrapnel from shattered wheels. A repeat violation exists when an employer previously has been cited for the same or a similar violation of a standard, regulation, rule or order at any other facility in federal enforcement states within the last five years. The Army had been notified by OSHA's area office in Frankfort, Ky., about these same violations at a Kentucky installation ….”
