1. Copy of applicants Passport

Requirements

a. All pages are required. Please ensure copies are clear and readable.

NOTE The validity of the passport must be more than 6 months upon submission of application to eXpats Service Centre.

The passport must have minimum of 6 consecutive empty pages.

2. Passport Sized Photos

Requirements 2 passport size (3.5 cm x 5.0 cm) with BLUE background.

3. Immigration Form: DP 11A

Requirements Please provide complete form.

4. Copy of Marriage / Birth Certificates

NOTE

If is in other languages than English, please get it translated by the authorised agency i.e Notary Public / Commissioner of Oath from respective country

CHINA NATIONALITY Please get it translated by the authorised agency and certified by the Malaysian Embassy / High Commission in China ONLY.
