Two kinds of urinary catheter:

Straight catheterFoley or retention catheter

Purposes:

To relieve discomfort due to bladder distention or to provide gradual decompression of a distended bladderTo assess the amount of residual urine if the bladder empties incompletelyTo obtain a urine specimenTo empty the bladder completely prior to surgeryTo facilitate accurate measurement of urinary output for critically ill clients whose output needs to be monitored hourlyTo provide for intermittent or continuous bladder drainage and irrigationTo prevent urine from contacting an incision after perineal surgeryTo manage incontinence when other measures have failed

III. Procedure

1. Explain the procedure to the patient and why it is necessary

2. Wash hands3. Provide for client privacy4. Place the client in the appropriate position and

drape all areas except the perineum5. Establish adequate lightning

6. Open the drainage package and place the end of the tubing within reach7. Open the catheterization kit8. Place a waterproof drape under the buttocks (female)9. Apply sterile gloves10. Organize the remaining supplies11. Attach the prefilled syringe to the indwelling catheter inflation hub and test the balloon12. Lubricate the catheter and place it with the drainage end inside the collection container13. Cleanse the meatus14. Insert the catheter15. Hold the catheter with the nondominant hand

16. For an indwelling catheter, inflate the retention balloon with the designated volume

17. Collect a urine specimen if needed18. Attach the catheter to drainage bag using sterile technique19. Examine and measure the urine20. Secure the collecting tubing to the inner thigh for females21. Wipe the perineal area of any remaining antiseptic or lubricant22. Return the patient to a comfortable position23. Discard all used supplies in appropriate receptacles24. Wash hands25. Document the catheterization procedure, including the catheter

size and results

Removing Retention Catheters:

1. Ask the client to assume a supine position as for a catheterization2. Remove the tape attaching the catheter to the client, don gloves, and then place the towel between the legs of the female client3. Insert the syringe into the injection port of the catheter, and withdraw the fluid from the balloon4. Do not pull the catheter while the balloon is inflated5. After all the fluid is withdrawn from the balloon, gently withdraw the catheter and place it in the waste receptacle

6. Dry the perineal area with a towel7. Remove gloves8. Measure the urine in the drainage bag, and record the removal of the catheter9. Following removal of the catheter determine the time of the first

voiding and the amount voided during the first 8 hours. Compare this output to the client's intake.
