Transition services are appropriate for clients that:• Have chosen to pursue curative treatment

• Want to remain in their own home with support

• Don’t want or qualify for hospice care

• Have a life limiting diagnosis

• Feel overwhelmed by complex medical decisions

• Have had increased calls or visits to the physician

• Have been hospitalized or needed emergency care several times in the past year

• Have changes in their ability in caring for daily living needs such as getting dressed, preparing meals, walking and bathing

The Transitions program provides emotionalsupport and practical assistance

(non-medical) to individuals and familieswho are coping with a life-limiting illness.

• Assessment and professional resource linking by a Social Worker• Telephone Support (bi-weekly Wellness Calls)• Companionship from a Trained Volunteer• Assistance with errands and transportation when available• Assistance connecting with additional community resources
