• Australian Unity will operate the Home Care service of NSW by early 2016

The Commonwealth and NSW provide funding to service providers.
Currently… Under the NDIS…
People with disability apply directly to service providers for funded packages.
The NDIS provides funding and supports to individual people with disability.
People with disability will have greater choice over the supports they need, who they get them from and when.
The funding amount is based on the types of supports and the agreed outputs the services should deliver.
The funding amount is based on the assessment of the individual’s support needs, and what is reasonable and necessary.
Supports are delivered by programs with different entry and eligibility criteria.
People can access the NDIS if they meet the disability or early intervention requirements.

From 1 July 2017 People will be able to access the NDIS from 1 July 2017, if they live in: Illawarra Shoalhaven Mid North Coast Murrumbidgee Northern NSW
South Eastern Sydney

People will be able to access the NDIS from 1 July 2016, if they live in: Central Coast Hunter New England Nepean Blue Mountains Northern Sydney

* The geographical approach to the rollout is based on existing districts used by the Department of Family and Community Services and NSW Health.
People can access the NDIS based on where they live and what supports they receive
From 1 July 2016
From 1 July 2017
People can access the NDIS based on where they live and what supports they receive
From 1 July 2018, all eligible people living in NSW will be able to access the NDIS.
People receiving specialist disability supports Includes people living in supported accommodation, accessing a community access service such as a day program, or case management service.
The majority will be able to access the NDIS in the first six months of each of the two year rollout periods. They will not need to apply for access to the NDIS, but will go through a simplified access process.
People who access specialist disability supports from time-to-time, or for a short amount of time each week Includes people who access respite or a community care program.
Will be able to access the NDIS throughout each of the two year rollout periods. People receiving respite services will go through a simplified access process. People receiving community care services will be supported to apply for access to the NDIS.
New participants People who do not currently receive specialist disability supports from the NSW Government.
Will have the opportunity to access the scheme when it starts in their local area, if they are in immediate need of assistance, and do not currently have support.

Post School Programs assessment process
Eligibility Assessment • Schools identify students with disability who are over 17 years
of age and in Year 12 who are eligible to complete the Post School Program Eligibility Assessment.
• Eligibility is determined based on an assessment undertaken by a Support Teacher Transition or Assessor in term 2 in school
• Evidence based tool – identifies how a young person manages activities of daily living
• All assessors are trained by ADHC in how to conduct the functional assessment
• Consent is requested before the assessment takes place

NSW School Leaver Eligibility Assessment process
Post School Programs and the NDIA trial sites
• ADHC is in negotiation with the NDIA about how school leavers eligible for Post School Programs will transition to the NDIS
• In the Hunter trial sites of Newcastle, Lake Macquarie and Maitland, the process has been well established.
• As LGAs transition, students have been either directly referred to the NDIA for assessment or the Post School Program assessment is shared with the NDIA
• School leavers have been prioritised for support under the NDIA to ensure they receive services

Roll out of the NDIS
• In 2016 ADHC plans to continue the post school program assessment process as usual in collaboration with Education Authorities – Year 1 Districts will participate in the PSP assessment
process and (with consent) the assessment will be shared with NDIA to form part of the NDIS planning
– NSW & NDIA will work together to ensure continuity of
support for eligible young people
