Internship Report - Requirement
• Any supplementary materials or appendices should be compiled into the Report as ONE document.
• Endorsement Form on the use of photos signed by your supervisor must be handed-in with your internship report in PDF.
• File size of the Report should NOT exceed 10MB. Reports must be submitted in PDF, DOC, or DOCX format only.
• Please submit your report by 4th September 2016 (Sunday) 11:59pm.

• Part A should be answered by team, while Part B and Part C should be answered by each member of the team on individual basis.

• At the time you submit the Report, each team please attach with 2-3 photos (jpeg format) with INTERNS at service, taken with service users, supervisors, etc. which might be used for Program promotion in the future.

Internship Report – Team with 2 interns
• The internship report should be submitted in team of 2 interns (if you have 2 interns in the same placement organization) conducting internship at the same placement organization.
• Each of you will write one report and combine two reports into one during submission.
• Therefore, each team should submit ONE report and ONE Photo Consent Form only.
