Waiver Read carefully before signing:
I know that running and/or walking a road race is a potentially hazardous activity. I should not enter and run or walk unless I am medically able and properly trained. I agree to abide by any decision of a race official relative to my ability to safely complete the course. I assume all risks associated with running and walking in this event, including, but not limited to: falls, contact with other participants, the effects of the weather, traffic and conditions of the road. All such risks being known and appreciated by me. Having read this waiver and knowing these facts and in consideration of the entry, I hereby for myself, heirs, executors, and administrators waive any and all claims I may have for damages against the Park Campus Wolves Den, school and Community Consolidated District 46 and all sponsors, and individuals associated with the event, their representative and successors and assignees for any and all injuries suffered by me in connection with this event, including pre and post-race activities. I hereby grant permission to Park Campus Wolves Den and its authorized agents to use my name and photographs, videotapes, motion pictures, recordings or any other record of my participation in this event for any purpose. Park School’s Inspiration Day is rapidly approaching! There are several great presenters scheduled to facilitate wonderful workshops for our children. Besides great volunteers to help on May 24th at the school, we are also in need of MANY items for our presenters to use as students participate on this inspiring day! Please consider donating an item or items from the list below by May 14th. Some items we need you may have extras lying around the house, the company you work for may be willing to donate, or you may be willing to purchase. Any quantity is acceptable and very much appreciated. Some quantities are not listed because several of a given item are needed. Donations can be dropped off to Aubrey at 477 West Meadow Mist Lane in Prairie Walk on or before May 14th. There is a collection bin on my front porch for your convenience. Note: Please do not drop off items in the school office unless arrangements have been made in advance because donated items will be counted and organized for I-‐Day prior to May 24th activities!
