������������ ��������������In order to become certified organic, livestock producers must demonstrate to an accredited certi-
fier that their operation complies with National Organic Program regulations. This process is begunby completing an Organic System Plan(OSP)—normally part of the applicationfor certification. The OSP illustrates tothe certifier how the producer plans tocomply with the regulations by detail-ing the practices, the inputs, and themonitoring procedures that will be used.The Organic System Plan is backed upby on-site inspection to ensure that theproducer is, in fact, farming or ranchingin the manner outlined in his or her OSP.It is the inspector’s responsibility to lookfor documentation and indicators thatsubstantiate the producer’s claim to or-ganic status, as well as look for any vio-lations.
The forms in this package are pro-vided as tools that livestock producerscan use for documenting practices, in-puts, and activities that demonstratecompliance with regulations or that as-sist in other aspects of farm record keep-
ing. The forms can be kept anywhere they make recording easy, such as the truck, milk house, ormachinery shed.
Please note that these are not required forms! Organic livestock producers have more thanenough mandatory paperwork to keep them occupied. These forms are merely intended to give yousomething convenient and organized to record routine things that may be important to document.Use only those forms that suit your operation and recycle the rest.

A. Pasture / Paddock / Hayfield Activity Log– use to record all practices and equipmentused, as well as the amount of forage pro-duced or hay harvested, for each field.

C. Organic Seed and Sprig Record – use fordocumenting source, treatment, and otherinformation on seed and sprigs used.
D. Organic Seed and Sprig Search Record – useto demonstrate attempts made to locate anorganic source when non-organic seed orsprigs are used.
