Anstieg von CGRP Blutspiegel während spontaner Cluster gemessen in der ipsilateralen

Behandlung mit O2 und sc sumatriptan reduzierten den CGRP Blutspiegel auf normal
durch NO-induzierte Cluster Attacken stieg der Plasmaspiegel von CGRP und wurde
durch sc sumatriptan wieder normalisiert
NO und CGRP interagieren bidirektional:

CGAL - study Conclusions
In individuals with episodic cluster headache, galcanezumab 300 mg SC:
- significantly reduced weekly cluster headache attack frequency across w1 to w3 - resulted in a significantly greater percentage achieving a ≥50% reduction in the weekly
cluster headache attack frequency at w3- resulted in a significant improvement on the PGI-I at w4, but not w8
Low incidence of discontinuation due to adverse events, with no clinically meaningful differences between galcanezumab and placebo for safety parameters except for a higher incidence of injection site pain with galcanezumab
CGAM - Chronic Cluster Headache Study Findings
- Phase 3 study of galcanezumab was conducted in patients with chronic cluster headache
- 3-month double-blind placebo-controlled period followed by an optional 12-month
open-label extension for completers of the double-blind period.
- In the placebo-controlled period, participants were randomized to galcanezumab 300 mg
(N=117) or placebo (N=120) subcutaneously once monthly for 3 months.
Primary endpoint not met - the mean change in weekly cluster headache attack frequency
across the 3-month double-blind placebo-controlled period was -5.4 for galcanezumab
vs -4.6 for placebo (p=0.33)
Zusammenfassung
• Humane, monoklonale Antikörper wurden speziell für die Migräne-Prophylaxe
entwickelt - Schneller Wirkeintritt und anhaltende Wirkung
• Signifikante Reduktion der Migränetage bei chronischer und episodischer Migräne
• Verträglichkeit auf Placebo-Niveau
• Einfache Injektion mittels Fertigpen, einfache Infusionslösung
• Geringes Potential für Immunogenität und neutralisierende Antikörper (vollständig
humaner monoklonaler Antikörper)
• Möglicher positiver Effekt beim episodischen Cluster-Kopfschmerz
