NOTE: EIDM has retained the PQRS naming convention for roles associated with QPP submission

2 Steps to Access QPP Portal
1. Create an EIDM AccountEIDM Account would exist if you’ve accessed: • Quality Reporting Use Reports (QRUR)• PQRS Feedback Reports in past• Submitted PQRS via Qnet Portal in past
2. Register for the Physician Quality and Value (PV) Application• Roles: Approvers and Submitters• Remote Identity Proofing (RIDP)• Multi-Factor Authentication (MFA)
*Call QPP Helpdesk at 866-288-8292 and provide group’s TIN/name of practice to inquire IF an EIDM
account exists.

• Review Provider(s) in Provider Enrollment Chain and Ownership System (PECOS).

New User Registration: Before You Start
• Legal Business Name (LBN)
• Medicare Billing TIN(s)
• Clinician’s Full Name(s)
• Rendering Physician’s NPI and PTAN
• Basic demographics: address, city, state, zip and phone
• Security Official role requires NPIs and PTANs for 2 different clinicians who bill under TIN*Access the EIDM User Guide for further details

What is a PTAN?

Provider Transaction Access Number-PTAN
• Medicare-only number issued by Medicare Administrative Contractors (MACs) upon enrollment in Medicare.
o MACs issue an approval/notification letter, including PTAN information, when an enrollment is approved.
• While only the National Provider Identifier (NPI) is submitted on claims, the PTAN is a critical number directly linked to a provider’s or supplier's NPI.
• A PTAN's use should generally be limited to a provider's communication with their MAC.

NPI/PTAN Relationships
• If a group NPI enrolled in Medicare, the group would be issued a PTAN.
• If an individual NPI enrolled in Medicare, the individual would be issued a PTAN.
• If an individual NPI enrolled in Medicare under multiple TINs, multiple PTANs would be issued by the MAC under those TINs.
• When creating a new, individual provider association in the Physician Quality and Value Program in the EIDM portal, the PTAN issued to the individual NPI should be entered.

Multiple PTANs-Locality
• More than one PTAN may be issued depending upon the reasonable charge locality, under the Medicare Physician Fee Schedule (PFS) locality configuration.
o Example: If a NPI practices under a single TIN but has a practice in John Wayne County (PFS Locality 01) and a practice in Green County (PFS Locality 99), multiple PTANs may be issued.

How to Find a PTAN
Option 1: • Find PTAN on the notification letter sent by the MAC when the
Medicare enrollment was approved

Ownership System (PECOS)

• For Initial Registration, next to Select a Group, choose Provider Approver.
• Next to Select a Role Description, choose Individual Practitioner for solo practitioner OR Security Official for a Group (2+ EP’s). Click NEXT.

Identity verification cannot be completed for users with a credit freeze:
Common issue due to recent Experian breach
An EIDM Manual Identity form would need to be completed, with the following documents:
• Driver’s license/state ID
• Passport
• Two statements from financial institutions addressed to the user at the address provided during registration

Completing Role Request
• Review request to confirm all the data was entered accurately
• If accurate, select ‘Submit’
• If a change needs to be made, select ‘Edit’ and make appropriate changes

• A tracking number will be displayed on screen, select OK

• Retain tracking number until the requested role has been applied to the
account

Important Tips
• Please be sure to create the new Organization OR Individual
Practitioner using the provider(s) Individual NPI and PTAN
combination(s).
• If needing to verify this information, contact your Medicare Part B
Contractor (MAC).
• Approver(s) will receive an email notifying them of request approval.
• Approver will need to log into the CMS Enterprise Portal to approve
or reject request.
• Notification of approval, denial, or other requests will be sent to the
role requestor’s email address on file for request.

Q. What if my EHR/Health IT Vendor is submitting my MIPS data for me, do I still need an EIDM Account?
A. Yes. It is recommended that providers obtain an EIDM Account, to view their data submitted on their behalf by a Qualified Clinical Data Registry (QCDR), CMS Qualified Registry, or EHR/Health IT Vendor.

Q. Can my vendor or third party intermediary access my final performance feedback?
A. It depends on their EIDM access.
• Yes, if your practice’s Security Official approved an EIDM role request for your organization from a health IT vendor or third-party intermediary to submit data on behalf of your practice.
• No, if your vendor (qualified registry or Qualified Clinical Data Registry) had an EIDM role for their own organization to submit data on behalf of your practice.

Q. What if I have more than one TIN in my organization, do I have to create separate EIDM Accounts?
A. No, you do not have to create separate EIDM Accounts if you have more than 1 TIN in your organization. Once you’ve created your primary EIDM Account, you can add “Associate to an Existing Organization”. A Role Request will be sent to the Security Official for approval.

Q. What if CMS is unable to verify my identity?
A. If your identity cannot be verified, you’ll need to contact your QPP Helpdesk and complete a manual identity verification process. You would complete a special form provided to you by CMS, and submit one of the following as proof of identity: drivers license, Federal or State photo ID, or U.S. passport, and two financial institution official bills addressed to you at the address entered in the registration process.

A. You are recommended to verify your providers enrollment information with PECOS for any errors.

Q. What if I am in an ACO, do I still need an EIDM Account?
A. Yes, groups participating in a Medicare Shared Savings Program (MSSP) ACO must obtain an EIDM Account and Role under their participant TIN to submit and meet ACI requirements under MIPS for purposes of MIPS APM scoring standard and MSSP ACO-11 quality measure.
