Increase in ETCO2 Decrease in ETCO2
• Increased muscular activity (shivering)• Malignant hyperthermia
• Increased cardiac output (during resuscitation)
• Bicarbonate infusion• Tourniquet release
• Effective drug therapy for bronchospasm
• Decreased minute ventilation
• Decreased muscular activity (muscle relaxants)
• Hypothermia
• Decreased cardiac output (cardiac arrest)
• Pulmonary embolism
• Bronchospasm• Increased minute ventilation

• Infrared Absorption•CO2 molecules absorb infrared light energy at specific wavelengths•An infrared beam passes through a gas sample containing CO2
•A ratio of light absorbed to light transmitted to the light which reaches the photodetector is reported as ETCO2

• Solid State vs. Chopper Wheel
Solid State CO2 Sensors No moving parts -
durability Use beam splitter to
measure IR light at two wavelengths
IR light source electronically pulsed
Chopper Wheel CO2 SensorsSpinning wheel - very fragileSpins to change parameter
measured by photodetector Gas sample to be measured
(data) Sample plus sealed gas
reference cell No light at all

Plotting CO2 vs time: is this end-tidal CO2?
• End-tidal CO2 may not reflect alveolar CO2 level
• No information on volume of CO2
• Dead space measurements are not possible
• Shape of capnogram may be misleading: a flat alveolar plateau may not be flat; inhalation and exhalation may not reflect effective ventilation

B-D: Exhalation
