Aging and dementia
a carepath has been developed for the dementia client – the special regional dementianurses train home-care teams to proceed
according to this model using as an example a real demanding case of a client in the team
education – lessons to the personnel, for example how the encounter the
demanding dementia client, stimulus activity for the dementia client, how to inform and collaborate with the relatives, how to notice and respect the autonomy of the client etc.
– multiprofessional meetings with the physician of the team, dementianurse and with other collaborators (e.g. geriatrician, social worker, physiotherapist) dealing with demanding clients in team
