Background
Indonesia is an
archipelagic country, or in other
words in Indonesian territory
consists of thousands of islands
scattered in various places.1
Maluku or internationally known
as the Moluccas is one of the
provinces based archipelago in
eastern Indonesia. Based on
satellite imagery LAPAN
identification, Maluku province
consists of 1,412 islands with an
area of each island varied.2,3 In
the small islands there are also
areas that are remote or far from
the city center. It becomes an

influence on the lack of access
that can be felt by people in the
area. One of them is education
access.
Geographical conditions of
the area to be one factor that
gives the effect of the lack of
quality education standards in the
Moluccas. This becomes a
problem that not only the
responsibility of the central
government but all of society,
both the teachers as primary
educators, and families as the
primary educators, private
institutions, health workers and
others.
In terms of health, too, the
people as retarded or limited
education as a result of the length
of the span of control that led to
access to health care can not be

apply it. There are still many
remote areas inaccessible so that
they do not know about it. In
addition, the quality is not

assigned to the remote areas for
various reasons, school buildings,
equipment for teaching and
learning.4 As health workers in
the narrow sense of a doctor who
is born of the Faculty of Medicine,

islands including remote areas
such, has a responsibility in
addition to helping and treating
patients also become educators
for children or people who are not
familiar with education in the
area.
Based on the description above
and which has been described in
the background, then the problem
studied are as follows:
1. What about quality
standards of education in
the area Moluccas?
2. What is the role of the
doctor as an educator in the
Moluccas?
3. What is the role of
development efforts and
compliance-related
competence as a physician
educator in the Moluccas?

Discussion
Education in the Region
Maluku Islands
Education is a basic
capital as the builder of human
qualities. Education is also a
requirement that is equivalent to
other necessities for the progress
of a nation.5 In this regard,
Indonesia has not fully meet the
quality standards of education.
Because there are still many
remote areas in Indonesia that
has not been touched by the
needs. The state of education in
Indonesia is still very sad.
Various problems became the
basis of the low quality of
education in Indonesia today.
Geographical barriers become
one of the problems in the
provision of quality education
services throughout the Context
Indonesia.1 Related offending
Indonesia which is the island
nation. Indonesia consists of
thousands of islands which are
difficult to reach so as to meet the

educational services in these
areas is quite difficult. One of the
provinces in Indonesia are related
to education issues, namely the
islands of Maluku. Maluku
consists of small islands which
are difficult to reach, because
until now it has not been evenly
education in Maluku. This
resulted in the declining quality of
education in Maluku from year to
year, ranging from primary school
to university.4 This proves that the
lack of control and monitoring
functions as well as the lack of
educational equity by the
Provincial Government. Thus the
need for the efforts of the
Provincial Government to
overcome this. With more focus
and attention to controlling
access to education in more
specialized areas in areas
beyond the reach or remote
areas. As well as equity and
expanding education for all the
different segments of society both
economically, gender, location of
residence, the level of intellectual
ability and physical condition.

In addition, the quality of
teachers in Maluku also fairly
poor. Based on Teacher
Competency Test in 2012, a
score of competence of teachers
in Maluku finished last or the
bottom of the 33 provinces in
Indonesia. sounds very bad,
because it will have an impact on
learners. Therefore, it is important
also to pay attention to the quality
of teachers as educators. Then
also in terms of uneven
distribution of teachers across the
region in Maluku. This can be
seen in remote areas that are not
teachers as educators. This
problem must be resolved.
In addition, the
infrastructure for the needs of
education, absence of
infrastructure or there, but not
worth it. These problems lead to
increasingly poor quality of
education in the Maluku islands.
In fact there are still remote areas
that are not familiar with
education.
Not just stop there, the
problem also affects the health
problems in the area of the
Maluku islands. Due to lack of
knowledge about health
education. So it can be seen also
how the quality of healthcare in
the Moluccas. With the
emergence of many new
diseases and deaths due to a
disease that is increasing.
Similarly, public education,
lack of knowledge or health
education also resulted from
ketidakadanya equalization of
health workers, especially doctors
in all regions of the Moluccas,
more precisely in the remote
areas. In the meantime, the
provincial government is trying to
try to optimize the health
personnel to be assigned to
remote areas in the Maluku
islands. As is also another
obstacle, namely the lack of
interest of health workers to serve
in Maluku due to geographical
conditions in the Maluku region
consisting of small islands.
As a physician educator in
Maluku Islands Region
The term comes from the
Latin doctor "Docere" which

means that a doctor is also in
addition to the duties and
responsibilities in terms of health
care practices, it can act as a
teacher or educator. Based on
the results of the developed
conceptual "The Bridging Project"
which is a combination of high

developed and established as a

overall to all people without
distinction of class society, both
economically, gender, until his
physical condition, and without
asking for any compensation. As
has been clearly conveyed in

that a doctor will teach science

agreement.10
As a physician who will be
placed in charge to small islands
up in remote areas, various roles
must be done. Not only as a
servant in health practice, but
also a doctor can be an educator.

Based on medical ethics, one
point common obligation of a
doctor is to consider the interests
of society and all aspects of
health care, and strive to become
an educator and a servant to the
people with the best-baiknya.11 In
the context of this paper, a doctor
educate or teach people about a
knowledge that he knew, not only
health but also education in
general education. The first step
that can be done is to teach the
little things like teaching children
in the areas of reading, writing,
science as a natural science,
social science, mathematics,
Indonesian or English. Then also
on health education in this
instance as proper handwashing,
brushing teeth and so forth.
When all the small things that can
be applied, then the next task of
the physician is able to teach
other things more complicated
regarding public health education
ie like environmental sanitation,
immunization of children,
nutritional needs, family planning

programs, or it could be about a
disease. Because of these things
that in the most remote areas,
people do not know about it. So it
can be seen high mortality rates
in remote areas. This is likely due
to a dirty environment, children
who are not immunized, or family
members that much because
they do not follow the family
planning program coupled with
low economic that can occur
malnutrition or other disease or
until death. Those things are like
a vicious circle that will not break
if there is no response to the
introduction of education in
general and health education to
the community. Therefore, the
role of a doctor in a remote area
is very important.
But besides that, it should
also be known that to be an
educator in this case a doctor,
many obstacles that must be
faced, even more so that in
remote areas where people are
not familiar with education. It is
difficult for how to make them
interested in education and try to
apply them in their daily lives.

Similarly with education on
health. Where different
understanding can allow
constraints with respect to
various problems activities and
habits that have become a
tradition in this case related to
health. Or also the cultural
background of the different local
communities. In this case, the
communication language is one
of the obstacles in carrying out
the functions or duties as a doctor
somewhere. In general, most
communities in Maluku had been
using the Indonesian language
but not disangkalkan that there
are also people who use local
languages in communication. In
general, communication is a
process of delivering thoughts or
information from one person to
another through a certain way so
that other people understand
exactly what is meant by a
transmitter of thoughts or
information.12 The principle is a
very important role in the
interaction between the physician

skill or ability in terms of
communicating.
There was also the need
for good management approach
from a doctor that can be done
with the psychological support
that makes people in remote
areas who felt supported and
believed in the doctor.
As one example of the
doctor's role as an educator is a
physician who became a lecturer.
Where is the knowledge that he
has, he teaches and educates
students to also could be a
professional and competent
doctors who would also seem to
be educators. And this will
continue to recur as such.
Development efforts and
Compliance Related Roles
Competencies Doctors For
Educators Regional Maluku
Islands
As a result of conceptual
"The Bridging Project", the doctor
as an educator is the first role
that was developed and
established as a doctor's

competence, to the need for
efforts to further develop this
competency. With more stressed
and taught since the beginning of
the medical students on public
health as well as train good
communication skills. So far, in
particular the students of the
Faculty of Medicine, University of
Pattimura have applied the
teaching concerning public health
education and the doctor of the
island in connection with the
condition of the area in the
Maluku islands. In reality there is
not a result of the application of
this teaching. But still expected
and prioritized doctors born of the
Faculty of Medicine, University of
Pattimura Ambon can be
competent doctor, not only in the
service of health practices but
also in terms of as an educator
and so forth.
Based on the
professionalism of education
development program by the
Indonesian Doctors Association
(IDI), as a doctor is required to
master the competencies like a
doctor in this case to obtain
registration letter physician. And
one of the requirements to fulfill
the terms of a certificate of
competence is a letter of
recognition of a physician's ability
to carry out medical practice
throughout Indonesia issued by
the College of Family Physicians
and Physician Indonesia
(KDDKI).13 The registration
certificate is valid for 5 years and
then re-registration must be done.
Back again to the requirements,
the physician must obtain a
competency certificate re
indicating that doctors are
maintaining their competence and
improve knowledge as the
development of science in the
field of medicine. With this
program, it is expected Maluku
Provincial Government can also
memperhatinya doctors in the
area related to the Maluku islands
to implement this program as a
development and fulfillment of
physician competence related
role as an educator in the Maluku
islands.

Conclusion
1. Education in the Moluccas as
a builder of quality human
capital base is still not fully
meet the appropriate quality
standards. There are still
many areas that are not
familiar with education
because of the geographical
conditions of the region in
Indonesia, which consists of
small islands that are difficult
to reach as well as the
distribution of teachers is
uneven, and the absence of
infrastructure for educational
needs. Not in spite of this, the
quality of healthcare in the
area of the Moluccas is also
fairly minimal. Originated as a
result of the lack of public
knowledge about health
education. And all these
problems back to the
geographical conditions in the
region of Maluku archipelago
consisting of small islands.
However, it is also proved that
the lack of control and
monitoring functions as well
as equity ketidakadanya
educators throughout the
region the Maluku islands.
2. The role of a doctor in the
Maluku islands not only as a
waitress health practice, but
also as an educator. In this
case a doctor give teachings
or educate both individuals,
families, and communities in
the region will be the
knowledge that he knows,
whether it is general
knowledge, natural science,
social science, mathematics,
Indonesian, or English. It is
also in accordance with the
profession, doctors teach
health education to the
community such as
environmental sanitation,
nutritional needs, bring
children for immunization,
family planning or
implementing programs also
teach a particular disease. All
of this is supported by the
skills or the ability to
communicate from a doctor
and a good management
approach with individuals,

3. Efforts related to the
development and fulfillment of
the role of a doctor's
competence as an educator in
the area of the Maluku islands
so far has been fairly good.
Due also to have the Faculty
of Medicine in Maluku then as
an initial effort is to teach
students about the ability to
communicate, and also
related to public health
education and the doctor of
the island in connection with
the vision of the Faculty of
Medicine, University of
Pattimura this as a
development-oriented medical
education of the island. It is
fairly good initial effort. The
reality is there are no results
from these initial efforts, the
doctors were born of the
Faculty of Medicine,
University of Pattimura this
other than as a waitress
health practice as well as
educators in remote areas in
the Maluku islands. However
prioritized and expected future
doctors who are born of the
Faculty of Medicine,
University of Pattimura can
execute their role as an
educator. Moreover, in terms
of development efforts and
compliance-related doctor
physician role as an educator
has been programmed by the
Indonesian Doctors
Association (IDI) with the aim
to show that a doctor can
maintain competence and
increase physician knowledge
as the development of
medical science.
Suggestion
1. Need for policies and
measures of the government
to improve the quality of
education in the area of the
Maluku islands, with more
focus in terms of control and
monitoring of education in
areas that are difficult to
reach, as well as doing the
distribution educators are
evenly distributed throughout
the area in the Maluku

islands. Likewise, the entire
community contributed to the
goal in this case in order to
improve education in the area
of the Maluku islands can be
reached. Similarly, health
workers, in this case the
doctor as an educator.
2. As a physician in the Maluku
islands should be to interpret
the role. Not only as a health
praktiksi but also as an
educator. So in this case can
help support education in the
Moluccas special especially in
remote areas.
3. The parties in terms of health
and development efforts to
further enhance the role of
compliance-related doctor
physician competence as an
educator in the Maluku
islands. Pay attention to the
quality of doctors to monitor
the course of the program
established by the Indonesian
Doctors Association (IDI)
concerning the certification of
competence in order to
maintain proper competence
of a doctor as well as
increasing knowledge as the
development of medical
science.
