Introduction•Health disparities continue to grow within the community and the city would benefit from a wellness center especially targeted at vulnerable population such as the homeless. Through an environmental scan there is strong evidence to support the community wanting and needing access to health promotion services. The purpose of this presentation is to gather funding and support by explaining our vision, goals, and plan to develop a wellness center while sustaining viability and growth for the future.

Mission Statement• To provide homeless citizens the

opportunity to receive safe and professional health services while helping to promote self-responsibility and enable individuals to gain control over their own health and wellbeing.

Philosophy• All patients should have access to the

necessary tools to achieve an optimal level of health.

• Having access to appropriate, high quality, and accessible public health services aimed at maintaining one’s own health and decreasing morbidity and mortality rates.

• In offering the physical, emotional, and spiritual care for the homeless population in our community.

• In delivering a clean, safe physical environment that meets provisions for basic needs.

Philosophy• In providing a safe and positive work

environment for our staff that values and respects everyone’s contributions.

• We will continue to evolve as a company by listening and learning from our patients, staff, and leaders in the community.

• Recognizing and encourage leaders within the community who strive towards increasing health and wellness.

• Policymakers and community leaders should be informed about the importance of wellness and knowledge about current healthcare issues.

Organizational Goals• Form a wellness clinic that provides information

regarding health promotion, wellness, and encourages an optimal level of health to homeless residents within the community.

• Allow access to wellness resources through a mobile network for all residential homeless shelters in the community.

• Increase access to public health services at a centrally located facility that demonstrates the benefits of follow-up health visits to the homeless residents within the community.

• Increase the awareness of community members to the health disparities facing the homeless population.
