PA Board of Accountancy No disciplinary actions taken. Lancaster County Douglas Patrick Barton, certificate and license no. CA024866L of Brownstown, Lancaster County, was suspended for a period of two years, after one year, the suspension will be stayed in favor of probation, and levied a civil penalty of $4,000; because he had his authority to provide professional services as a certified public accountant by any other state or jurisdiction suspended. (1/21/20) Montgomery County Daniel Joseph Cunningham, license no. MV125871L of Harleysville, Montgomery County, was levied a civil penalty of $500; because he was convicted, entered a plea of guilty, or entered a plea of nolo contendere in a court of competent jurisdiction in this or any other state or Federal jurisdiction or forgery, embezzlement, obtaining money under false pretenses, extortion, conspiracy to defraud, bribery, odometer tampering, or any other crime involving moral turpitude, while his current license is in force or within five years prior to the application for or issuance of a license. PA Board of Auctioneer Examiners Philadelphia County David Weiss, of Philadelphia, Philadelphia County, was denied a license to practice auctioneering, based on not having an active Apprentice Auctioneer License. (11/05/19) PA Board of Barber Examiners Cumberland County Eric Manuel Rivera, temporary authority to practice no. TBL004360, of Mechanicsburg, Cumberland County, had his application to sit for the barber licensure examination granted and, upon successful completion of the examination, his barber license shall be granted and then immediately placed on probation for one year based on his criminal record. TBL004364, of Landisville, Lancaster County, had his application to sit for the barber licensure examination granted and, upon successful completion of the examination, his barber license shall be granted and then immediately placed on probation for no less than 2 years based on his criminal record. BM094524, of Allentown, Lehigh County, had his application to sit for the barber manager licensure examination granted and, upon successful completion of the examination, his barber manager license shall be granted and then immediately placed on probation for 2 years based on his criminal record. BO433133 of Bethlehem, Lehigh County, was levied a civil penalty of $1,000; must cease and desist from employing unlicensed individuals, and it received a reprimand on its disciplinary record with the Board, because it failed to employ licensed persons, and aided and abetted unlicensed practice activity in the practice of barbering. (12/19/19) PA Board of Chiropractic Fayette County Randolph Aaron Magone, license no. DC006269L and AJ00629L of Fayette City, Fayette County, was suspended for no less than three years, immediately stayed in favor of probation, because he is unable to practice Chiropractic with reasonable skill and safety to patients by reason of illness, drunkenness, excessive use of drugs, narcotics, chemicals, or any type of material, or as a result of any mental or physical condition. (1/16/20) PA Board of Cosmetology Allegheny County Tara Dunning, a/k/a Tara Marie Dunning of Pittsburgh, Allegheny County, license no. CO289545, was placed on probation for two years based upon felony convictions. (11/21/19) Cumberland County Jamie Lee Parker, license no. CO303281, of Mechanicsburg, Cumberland County, upon successful completion of the examination will be granted a cosmetology license on probation for no less than one year, based on her criminal history. (8/29/19) Lancaster County Justin A. Nickle, license no. CO264869, of Ronks, Lancaster County, was levied a civil penalty of $1000, based upon dishonest practice. (07/08/19) Philadelphia County Ana F. Liz, license no. CO264046, of Philadelphia, Philadelphia County, was assessed a $250 civil penalty for practicing cosmetology on a lapsed license. (07/19/19) Westmoreland County James W. Sanders, license no. CO282276, of Latrobe, Westmoreland County, was suspended pursuant to the Order of the Court of Common Pleas of Allegheny County dated August 13, 2018, which the court issued under section 4355 of the Domestic Relations Code. The suspension is effective immediately. (09/25/18) PA Board of Crane Operators

Out of State Brett C. Tedder, of Pickerington, Ohio, was denied a crane operator’s license based upon disclosed incidents involving alcohol and/or drugs. (12/16/19) PA Board of Dentistry Allegheny County Deron T. Kovac, D.M.D, license and certificate no.: DS027813L, of Pittsburgh, Allegheny County, was indefinitely suspended for no less than five years, immediately stayed in favor of probation. (01/17/20) Dauphin County Harold Joseph Harper, license no. DH008966L of Millersburg, Dauphin County, permanently voluntarily surrendered his license, because he failed to demonstrate qualifications or standards of licensure. (1/17/20) Philadelphia County John Y Kim, license no. DS024115L of Philadelphia, Philadelphia County, was suspended for two years, immediately stayed in favor of probation, levied a civil penalty of $2,500; and costs of investigation of $3,300, because he failed to conform to the standards of acceptable and prevailing dental practice, and failed to follow infection-control recommendations issued by the Federal Centers for Disease Control or to ensure that auxiliary personnel and other supervisees follow these Federal guidelines. (1/17/20) Tuyet-Ba Trieu, license no. DS024367L of Philadelphia, Philadelphia County, was indefinitely suspended, levied a civil penalty of $6,000; costs of investigation of $2758.45; and must complete at least 10 hours of remedial education, because she failed to conform with current infection-control recommendations issued by the Federal Centers for Disease Control or ensure that others followed the Federal guidelines; and engaged in unprofessional conduct (1/17/20) Joel Nash, D.D.S., license no. DS031203L, of Philadelphia, Philadelphia County, had his license publicly reprimanded and was assessed a $1000 civil penalty for making false statements on his biennial registration. (03/13/19) Wayne County Michael Carl Rogers, license no. DS026802L of Honesdale, Wayne County, was levied a civil penalty of $7,000; costs of investigation of $12,585; a reprimand was placed on his permanent disciplinary record with the Board, must complete at least 20 hours of remedial education, and must Cease and Desist from the administration of anesthesia outside the scope of the Restricted II Anesthesia Permit Number DN02602A, because he failed to conform to acceptable and prevailing standards of dental practice; and violated a regulation of the Board by relating to noncompliance requirements, incompetence, negligence or misconduct in rendering services authorize by a Restricted II Anesthesia Permit. Westmoreland County

Naghmeh Pashmini, license no. DS040219 of Greensburg, Westmoreland County, was levied a civil penalty of $3,500; costs of investigation of $2,998.31; had a reprimand placed on her disciplinary record with the Board, and must cease and desist from the administration of conscious sedation, deep sedation, and/or general anesthesia in the practice of dentistry until she receives the proper permit issued by the Board, because she engaged in unprofessional conduct by failing to possess a current permit issued

DS036967 of Meridian, MS was suspended, immediately stayed in favor of probation for a period concurrent with that imposed by the State of Mississippi, and was levied a civil penalty of $1,500; because he had a license to practice dentistry revoked, suspended or had other disciplinary action imposed by the proper licensing authority of another state. (1/17/20) Blane R Hamilton, license no. DS039027 of Mansfield, TX was levied a civil penalty of $1,500; and had a reprimand placed on his disciplinary record with the Board, because he had a license to practice dentistry revoked, suspended, or had other disciplinary action imposed by the proper licensing authority of another state. (1/17/20) Ean Rupert James, D.M.D., PA license no. DS035766L, of Longmeadow, Massachusetts, and of Cherry Hill, New Jersey had his license reprimanded and was ordered to pay a civil penalty of $2,500 because he had his license to practice dentistry disciplined by the state of Connecticut. (11/19/19) PA Board of Professional Engineers, Land Surveyors and Geologists Allegheny County Richard J Oleniacz, license and registration no. PE041907E of Pittsburgh, Allegheny County, had a reprimand placed on his disciplinary record with the Board, because he failed to disclose on two license renewal applications any finding or verdict of guilt or other criminal conviction since the most recent application for renewal. (1/08/20) Chester County Christopher S Binck, license and registration no. PE032532E of Valley Forge, Chester County, permanently voluntarily surrendered his license to practice as a professional engineer, because he pled guilty to a felony in a state or federal court, and has been convicted of a felony. (1/08/20) Montgomery County Jon E Thompson, license and registration no. PE048939R of Dresher, Montgomery County, was levied a civil penalty of $3,500; because he practiced the profession of an engineer without being properly licensed to do so. (1/08/20)

Out of State Matthew E Lane, license and registration no. PE074447 of Saint Johns, FL was levied a civil penalty in the amount of $1,000; because he practiced the profession of an engineer without being properly licensed to do so under the act. (01/08/20) John Edward Skwiot, license and registration no. PE083305 of Tempe, AZ was indefinitely suspended, immediately stayed in favor of indefinite probation; and was levied a civil penalty in the amount of $1,000; because he had a license to practice engineering, land surveying or geology suspended or revoked or having other disciplinary action taken for conduct relating to the practice of engineering, land surveying or geology, by the proper licensing authority of another state. Sweeny, Jr., FD, license no. FD014334L, of Olyphant, Lackawanna County, was issued a public reprimand for practicing fraud or deceit in obtaining the renewal of his license. (12/05/19) Lancaster County Andrew T. Scheid, license nos. FD013388L and FS015683, of Millersville, Lancaster County, had his licenses to practice as a funeral director and funeral supervisor temporarily suspended, and Andrew T. Scheid Funeral Home, license nos. FO013388L and FB014562, of Millersville and Lancaster, Lancaster County had their licenses to practice as a sole proprietor funeral establishment and branch funeral establishment temporarily suspended, pending a hearing, on the grounds that their continued practice within the Commonwealth may be an immediate and clear danger to the public health and safety. (01/16/2020) Washington County Karl Jackson, FD, license no. FD013616L, of Donora, Washington County, was issued a $1,000 civil penalty and was suspended and immediately stayed in favor of probation based on having been convicted of felony crimes involving moral turpitude. PA Board of Massage Therapy Erie County Tina Yu of Erie, Erie County, was granted a license to practice massage therapy, said license immediately placed on probation for no less than 3 years, based applicant’s agreement to enroll in the Professional Health Monitoring Program. (12/8/19) PA Board of Medicine Allegheny County Mark G. Boles, MD, license no. MD041044L, of Monroeville, Allegheny County, had his license publicly reprimanded and placed on indefinite probation to run concurrent with his criminal probation based on his felony conviction. MD039613L of Blackwood, NJ, permanently voluntary surrendered his license to practice as a medical physician and surgeon in the Commonwealth of Pennsylvania, because he had a license or other authorization to practice the profession disciplined by the proper licensing authority of another state. (12/17/19) Robert Schnitzlein, M.D., license no. MD463438, of Abingdon, MD, had his license to practice as a medical physician and surgeon temporarily suspended, pending a hearing, on the grounds that his continued practice within the Commonwealth may be an immediate and clear danger to the public health and safety. (01/17/20) Ronald Dolor Gonzales, M.D., PA license no. MD067057L of Chicago, Illinois had his license publicly reprimanded for having disciplinary action taken by a proper licensing authority of another state. (04/04/19) Tristram G. Horton, M.D., PA license no. MD438573, last known of Scottsdale, Arizona, had his license revoked, based on receiving disciplinary action by the proper licensing authority of another state. (01/15/19) Qing Liu, M.D., PA license nos. MD448697 and PAK000217, of Lynchburg, Virginia had his licenses publicly reprimanded for having disciplinary action taken by a proper licensing authority of another state. (06/25/19) Paula Marie Nelson, M.D. a.k.a. Paula Marie Nelson-Adesokan, M.D., PA license no. MD427315, of Lilburn, Georgia, had her license revoked based on unprofessional conduct and for having disciplinary action in another state. (11/15/19) Navigation Commission for the Delaware River and its Navigable Tributaries No disciplinary actions taken. Secretary of the Commonwealth – Notaries Public Dauphin County Roger J. Cackovic, notary commission no. 1021647, of Harrisburg, Dauphin County, permanently and voluntarily surrendered his notary commission because he failed to maintain an accurate notary register by failing to record the character of the act. (01/07/20) Lackawanna County Carole D. Colussy, notary commission no. 1029450, of Scranton, Lackawanna County, had her notary commission suspended for three months, immediately stayed in favor of probation; levied a civil penalty of $250; a probation assessment of $150; and must complete six hours of remedial education, because she engaged in conduct that demonstrates that the individual lacks the honesty, integrity, competence, or reliability to act as a notary public, in violation of RULONA, in that she failed to cross out all open or blank signature lines on the mortgage document prior to notarization. Williams, notary commission no. 1179337, of Scranton, Lackawanna County, had his notary commission suspended for six months, immediately stayed in favor of probation; levied a civil penalty of $250; a probation assessment of $300; and must complete six hours of remedial education, because he failed to be of good character, integrity and ability, and failed to ensure that no other person was able to use his notary seal, and in that he failed to keep and maintain custody and control of an accurate chronological register of all official acts performed by him. Philadelphia County Tobias L. Biddle, notary commission no. 1011576, of Philadelphia, Philadelphia County, had his notary commission suspended for three months, immediately stayed in favor of probation, and levied a probation assessment of $150; because he lacked the honesty, integrity, competence and/or reliability to act as a notary public, because he violated the terms of a previous Consent Agreement and Order. (12/19/19) PA Board of Nursing Allegheny County Mary Sue Deiley, RN, license no. RN264830L, of Clairton, Allegheny County, was indefinitely suspended, retroactive to September 23, 2019, and assessed $675 in costs of investigation, for being unable to practice professional nursing with reasonable skill and safety to patients and violating a Board order. (12/9/19) Pete Schmidt, RN, license no. RN515422L, of Pittsburgh, Allegheny County, had his license reinstated and immediately placed on probation for no less than three years, pursuant to the terms of a Board Order, based on his agreement to enroll in monitoring in the Professional Health Monitoring Program. (05/06/19) Jennifer N Legg, license no. RN618158 of Pittsburgh, Allegheny County, was suspended for an indefinite period with a minimum period of one year and thereafter to be served as a stayed suspension for as long as she remains on criminal probation, because she was convicted of or pled guilty to crimes of moral turpitude in the courts of this Commonwealth (1/24/20) Robert M Fabiszewski, license no. RN330704L of Pittsburgh, Allegheny County, voluntarily surrendered his license to practice as a registered nurse, because he was convicted of or entered a plea of guilty to a crime of moral turpitude and has been convicted of any felony. (1/24/20). Berks County Stephen J Szymanski, license nos. RN506924L and SP010457 of Shillington, Berks County, was suspended for a period of two years, with such suspension immediately stayed in favor of probation. levied a civil penalty of $500; and he must complete a total of at least 30 hours of remedial education. MV246912, of Elizabethtown, Lancaster County, was suspended for failing to pay a previously imposed civil penalty and costs of investigation. (12/20/19) Northampton County William A. Saavedra, license number MV216743, of Nazareth, Northampton County, was reprimanded and assessed a $500 civil penalty based upon his conviction for a crime of moral turpitude. MV269273, of Philadelphia, Philadelphia County, was granted a vehicle salesperson license on probation concurrent with his criminal probation based upon his past conviction. (12/12/19) Manley L. Pack, of Philadelphia, Philadelphia County was granted a vehicle salesperson license, license no. MV259979, and was immediately placed on probation through the completion of his criminal probation and payment of all costs, fines and/or restitution. MV269274, of York, York County, had her application for a vehicle salesperson license granted and placed on probation for a period of two years based upon her criminal convictions. (12/13/19) Troy E. Grissinger, license number MV204463, of Dover, York County, was revoked based upon his criminal conviction for felony crimes of moral turpitude. (12/12/19) Out of State Jesse D. Burrier, license no. MV269332, of Thurmont, Maryland, had his application for a vehicle salesperson license granted and placed on probation for an indefinite period of time based upon his criminal convictions. BV006195L of Ebensburg, Cambria County, was levied a civil penalty of $15,000; costs of investigation of $8,000; cease and desist from providing treatment of all cancers to

animals, and complete remedial education, because he engaged in incompetence, gross negligence, or other malpractice, and/or departed from, or failed to conform to, the standards of acceptable and prevailing veterinary medical practice; violated the Board’s regulation which states: “Veterinarians shall practice in accordance with current advancements and acceptable and prevailing standards of care in veterinary medicine, including using current proven techniques, drugs and scientific research that may affect treatment decisions. Veterinarians shall practice in accordance with advancements and acceptable and prevailing standards of veterinary medical practice in this Commonwealth related to the pharmacologic properties, indications and contraindications of drugs and biologics.”; failed to, while keeping a client’s animal while the practice is closed, inform the client, either orally or in writing, whether a qualified person would be on the premises and what level of monitoring the animal will receive during that time; failed to explain the benefits and reasonably anticipated significant potential risks of treatment options to clients; failed to provide documentary evidence of a patient's illness, hospital care and treatment and serve as a basis for review, study and evaluation of the care and treatment rendered by the veterinarian; and failed to maintain required veterinary medical records. (1/10/20) Delaware County Elizabeth Kathryne Daniel, license no. BV009373L of Media, Delaware County, was levied a civil penalty of $2,000; costs of investigation of $1,500; and must complete 12 hours of remedial education, because he failed to properly document surgical and dental procedures; and failed to practice in accordance with current advancements and acceptable and prevailing standards of care in veterinary medicine. (1/10/20) Erie County John Raymond Lyon, DVM, license no. BV003766L of Corry, Erie County, was levied a civil penalty of $3,000; costs of $3,149.12; and must successfully complete at least 13.5 hours of remedial education on the topic of animal fractures. (01/10/20) Jefferson County Marin Boghean, license no. BV011139 of Brookville, Jefferson County, was levied a civil penalty of $4,000; and costs of investigation of $1,000; because he violated a Board regulation which states “Veterinarians shall practice in accordance with current advancements and acceptable and prevailing standards in veterinary medicine including using proven techniques, drugs and scientific research that may affect treatment decisions;” and a regulation which requires that veterinarians serve as patient advocates especially regarding the alleviation of pain and suffering, consistent with the acceptable and prevailing standards of veterinary medical practice; and failed to maintain required veterinary medical records. (1/10/20) Lancaster County Mary Jane Potter, license no. BV008600L of Kirkwood, Lancaster County, was levied a civil penalty of $4.000; and must complete seven hours of remedial education on veterinary law and ethics, because she engaged in fraud, deception, misrepresentation and/or illegal practices in or connected with the practice of veterinary medicine. (1/10/20) Out of State Thomas P Lopez, license no. BV013374 of Wellsville, NY was levied a civil penalty of $750; and a reprimand was placed on his permanent disciplinary record with the Board, because he was disciplined by a licensing or disciplinary authority of another state or county for an act that would be grounds for discipline by this Board.
