Northampton County, was ordered to pay a $4,050 civil penalty and to cease and desist from failing to
have students supervised by a licensed barber teacher at all times, failing to maintain and/or produce student
records, employing unlicensed barber teachers, aiding and abetting unlicensed practice activity in the
profession of barber teaching, failing to have a sign indicating work is done on the public by students, failing
to adhere to equipment requirements to operate as a school, operating as a barber school at a new location
without updating the board and having that location inspected first, and operating an unlicensed online barber
school because they failed to be under the immediate supervision of a licensed barber teacher at all times,
failed to properly keep and produce student records upon request by an inspector/investigator, employed an
unlicensed barber teacher, aided and abetted unlicensed practice activity in the profession of barber teaching,
failed to have an “ALL WORK IN THE SCHOOL DONE BY STUDENTS ONLY” sign displayed near the
entrance, failed to have an adequate number of sinks in proportion to the number of workstation chairs,
operated as a school at a new location without having that location inspected to determine whether all
requirements to operate as a school have been met, and operated an online barber school without a permit or

Philadelphia County Andy Cupeles, license no. BL053459, of Philadelphia, Philadelphia County, was suspended pursuant to
the Order of the Court of Common Pleas of Philadelphia County dated September 22, 2015, which the

Philadelphia County Janet M. Dailey, F.D, license nos. FD013089L and FS013089L, of Philadelphia, Philadelphia County,
was revoked and a $180,000 civil penalty imposed for her engaging in gross incompetency, negligence or
misconduct in carrying on of the profession; engaging in gross immorality; failing to promptly dispose of
deceased bodies; failing to properly store human remains; demonstrating disrespect toward the remains of
the deceased; and failing to retain a copy of written agreements and records of transaction. (01/15/16).
Powell Mortuary Services, Inc., license no FR000247L of Philadelphia, Philadelphia County, was
revoked and a $120,000 civil penalty imposed for engaging in gross incompetency, negligence or
misconduct in carrying on of the profession; engaging in gross immorality; practicing the profession at a
location other than the location indicated on its license; and practicing on a lapsed license. (01/15/16)
Marcus W. Coleman, of Philadelphia, Philadelphia County, was assessed a $10,000 civil penalty and
costs of $1,269.27 for the unlicensed practice of funeral directing.

Allegheny County Micah J. Moffett, license no. MSG007794, of Duquesne, Allegheny County, had his license to practice
massage therapy temporarily suspended, pending a hearing, on the grounds that his continued practice
within the Commonwealth may be an immediate and clear danger to the public health and safety.

Monroe County Dawn L. Curtis, a.k.a. Dawn Lee Kanapinski, Dawn Woodward, d/b/a American Dolls, unlicensed, of
Tannersville, Monroe County, was ordered to pay a $7,500 civil penalty and costs of investigation of
$250 and to cease and desist from performing any activity requiring a license to practice as a massage
therapist in the Commonwealth of Pennsylvania; from performing any other activity which requires a
license, registration, certificate, authorization, approval or permit issued by the board unless and until the
board issues the appropriate authorization to practice the profession; agrees to never apply to the State
Board of Massage Therapy for a license to practice as a massage therapist in the Commonwealth of
Pennsylvania; agrees to permanently close the facilities of American Dolls, located at 611 Tannersville
Peddlers Village, Unit 15, Tannersville, PA 18372 and Everlasting Spa, located at 5224 Milford Road,
East Stroudsburg, PA 18302; permanently disconnect all phone numbers for both of the previously listed
businesses; remove from the names of both previously listed businesses’ names and from all
advertisements describing their activities, including but not limited to signage, brochures, business cards,
price lists, telephone listings, and internet sites, the words “massage,” “massage therapist,” “massage
practitioner,” “masseur,” ”myotherapist,” or any derivative of these terms and their related abbreviations,
which implies directly or indirectly that massage therapy services are being provided; agrees to never
open, own, operate, and or act as the responsible employee of any facility with the intent or purpose of

practicing massage therapy in the Commonwealth of Pennsylvania; because she held herself out as a
massage therapist, engaged in the practice of massage therapy without a license, and utilized the services
of unlicensed personnel to provide massage therapy services to customers of American Dolls. (04/12/16)
Montgomery County Christopher Strain of Jenkintown, Montgomery County, was granted a license to practice massage
therapy, said license immediately placed on probation for at least two years based on his prior criminal

Philadelphia County Lisa Dibiasio, unlicensed, of Philadelphia, Philadelphia County, was ordered to pay a civil penalty of
$2,000 and costs of investigation of $257.74 to cease and desist from hiring or employing any
individual(s) to perform any activity or service requiring a license to practice as a massage therapist in the
Commonwealth of Pennsylvania or from performing any other activity which requires a license,
registration, certificate, authorization, approval or permit (hereinafter referred to collectively as
“authorization to practice the profession”) issued by the board unless and until the board issues the
appropriate authorization to practice the profession and shall immediately remove from its business name
as well as from all advertisements describing its activities, including but not limited to signage, brochures,
business cards, price lists, telephone listings, and internet sites, the words “massage,” “massage therapist,”
“massage practitioner,” “masseur,” “myotherapist,” or any derivative of these terms and their related
abbreviations which implies directly or indirectly that massage therapy services are being provided
because the word "massage" was used in advertisements describing the services provided by Paradise Spa
which implies directly or indirectly that massage therapy services are being provided even though the
services of the business are not being provided by licensees, that Ms. Dibiasio, as the responsible
employee/officer of Paradise Spa, employed unlicensed personnel as massage therapists to provide
massage therapy services for customers of Paradise Spa, and, as the responsible employee/officer of
Paradise Spa, aided, assisted, and hired individuals to perform or offer to perform massage therapy
services for customers of Paradise Spa without being licensed by the board. (04/12/16)

temporarily suspended, pending a hearing, on the grounds that his continued practice within the

surrendered his license because he is unable to practice medicine with reasonable skill and safety by

Out of State Nedra Regina Dodds, PA license no. MD064336L, of Acworth, Georgia, was indefinitely suspended and
assessed a $500 civil penalty based on disciplinary action taken by the proper licensing authority of

Philadelphia County Jason Howell, notary commission no. 1218263, of Philadelphia, Philadelphia County, was suspended for
twelve months, with such suspension immediately stayed in favor of probation, subject to terms and
conditions as specified by the Secretary, assessed a civil penalty of $500 and, a probation assessment of
$600 for costs of continuing monitoring, and must successfully complete a six-hour notary practice and
procedure course during probation period because he failed to impose the proper fees, failed to keep and
maintain a proper register of all official notary acts, and failed to be familiar with the duties and

Allegheny County Joseph Michael Shehab, license no. RN506111L, of Natrona Heights, Allegheny County, voluntarily
surrendered his license to practice as a registered nurse because he is unable to practice the profession
with reasonable skill and safety to patients by reason of mental or physical illness or dependence on drugs

Ethelia C. Massie, license no. RN618029, of Turtle Creek, Allegheny County, was placed on probationary
status for a period of one (1) year retroactive and concurrent with a September 7, 2015, order suspending
