is a limited attendance, 1-day presentation for Dentists and up tofour staff members. You must complete the course registration form.The Registration fee of $2075.00 CAD includes one dentist and 4auxiliary staff, all course materials, breakfast, lunch, as well as fullparticipation in all other Winter Clinic activities. The registeringdentist must be a resident of Canada or the United States of America.

Cancellation and Refunds:

You will be issued a full refund, less a $25.00 administration feeuntil October 20, 2007. Between October 22 and October 26, 2007,a 50% refund. There will be no refunds after October 27, 2007.

Badges will be mailed to attendees who register on or before

Welcome to the 70th Annual Winter Clinic

It is with great pleasure that we welcome the entire dental community to the 70th Toronto Academy of Dentistry’s

Winter Clinic, to be held on Friday November 9, 2007 at the much larger Metro Toronto Convention Centre, South

Building.

The Hands on Experience remains an integral part of the clinic along with world class speakers and exhibits. Inno-

vations such as Dr. Robert Cowie’s “clicker” interaction presentation and amous chef Ken Kostick who will be

speaking on stress free entertaining will add to this year’s program. Web based handouts in a PDF format will be

introduced for the first time.

Those less fortunate in our community benefit from our Winter Clinic. Through the Toronto Academy of Dentistry’s

Endowment Fund, many organizations like the Children’s Aid Foundation, Bloorview MacMillan Children’s Foun-

dation, The Hospital for Sick Children, Kids Help Phone, March of Dimes, About Face, Daily Food Bank, Easter Seals,

the University of Toronto Faculty of Dentistry and the University of Western Faculty of Dentistry have all benefit-

ed greatly by donations made possible by your participation. We encourage you to purchase 50:50 tickets at the

He is responsible for instruction of appearancerelated dental treatments and dental photography. He is active in bioma-terial research in bonding materials, directs a study club in aesthetic den-tal treatments, and lectures and directs workshops nationally and inter-nationally on clinical aesthetic bonding treatments. About the ProgramThis presentation will review the materials and techniques available todentistry to give patients the smiles they want while conserving naturaltooth structure. The proliferation of bleaching techniques from over-the-counter to controlled in-office bleaching, direct composite placement,bonded ceramics, periodontal alterations and the materials to use themwill be discussed. Visual communications is becoming the standard ofcare expected by patients in the age of Internet to television to maga-zines. Every office now has to be experienced in digital imaging. Systemsfrom simple to advanced are available, but techniques which least inter-fere with practice is still most important.

About the ProgramThe dental curriculum is inarguably amongst the heaviest of any univer-sity program. About the ProgramRemakes of crown and bridge prostheses can be avoided or minimized byprecementation inspection of ceramic-based prostheses for evidence ofthermal incompatibility cracks, debris in the ceramic, inadequate exten-sion of opaque and/or body porcelain, incorrect metal or ceramic frame-work design, poor quality solder joints, defective cast-joined connectors,inadequate polishing or glazing method, and evidence of overfiring orunderfiring. This programme will guide clinicians in how to examine lab-oratory made restorations prior to cementation. Actual examples ofrestorations that demonstrate problems we may encounter, and previ-ously failed restorations that failure may have been avoided will be avail-able to examine.

About the Speaker Dr. Anusavice received a PhD degree in Materials Science and Engineer-ing from the University of Florida in 1970 and a DMD degree from theMedical College of Georgia in 1977. He served as an Assistant Professorand Associate Professor between 1973 and 1983 at the Medical College ofGeorgia. Dr. Anusavice is Chair of Dental Biomaterials at the University ofFlorida. He also serves as the Associate Dean for Research. In 2004 he wasawarded the title of Distinguished Professor at the University of Florida.He is the co-author and editor of three books, has published over 150research articles and 40 book chapters on the properties of dental bioma-terials and caries prevention materials. He has received continuousresearch grant funding for 27 years from the National Institutes of Health(NIH). Dr. Anusavice served as the President of the American Associationfor Dental Research (AADR) from 2003 to 2004. About the ProgramClinical decision-making is associated with great variability among den-tists. Decisions on when to place a restoration because of suspectedcaries lesions is one of the most difficult choices because of inconsistentphilosophies among traditional prevention, minimally interventive den-tistry, minimally invasive dentistry, preservative dentistry, and modernmanagement of caries as an infectious disease. About the ProgramThis session will provide an overview of current orthodontic-periodontalconsiderations. Evidence-based research has shown that the skeletal pat-tern is important to gauge a patient’s potential for facial gingival reces-sion. A prognosticator in evaluating periodontal risk that has been underappreciated is the transverse skeletal discrepancy. This discrepancy is alocal factor that can be corrected and reduce a patient’s potential for gin-gival recession. Skeletal relationships clearly identify patients who aremore susceptible to periodontal breakdown, exhibit unstable dentalcamouflage and lead to less than optimal dentofacial esthetics. Earlytreatment in the deciduous or early mixed dentition is ideal to correctorthopedically transverse skeletal abnormalities. In addition to prevent-ing periodontal problems correction of the transverse (bucco-lingual)skeletal discrepancy should be done to achieve greater dental and skele-tal stability in adult retreatment cases, in preparation of tooth replace-ment or implant placement, and to improve dentofacial esthetics seenwith maxillary transverse deficiency. About the Speaker Dr. Gerlach is Research Fellow in Worldwide Clinical Investigations atProcter & Gamble. His clinical trials research includes initiatives in theareas of caries prevention, periodontal therapy, and esthetic dentistry,especially tooth whitening. With over 100 studies, his research has led tonovel delivery systems for vital bleaching and new dentifrice formula-tions for stain control. Dr. Gerlach is a public health dentist who receiveda dental degree from the University of Michigan and a public healthdegree from the University of South Florida. His work experienceincludes various government and academic appointments, as well asextensive private practice experience.

About the ProgramPeriodontal disease is reported to be the leading cause of tooth lossamong adults. New research suggests the state of periodontal healthmay also have systemic implications. Clinical trials have been conductedto evaluate the relationship between oral health and cardiovascular dis-ease, diabetes, pregnancy, and others. Information about the oral-sys-temic link continues to be reported not only in the professional literaturebut also in the lay media. It’s important for dental professionals to helppatients differentiate science from hype as they are increasingly exposedto information, and in some cases misinformation, about the impact oforal health on overall well-being.

While the connections between oral and overall health are emerging, it isclear that preventive care is critical to help patients achieve and maintaingingival health. New technologies have emerged to provide patientswith safe and effective treatments for home care. Inthis capacity, he provides advice to the federal government on dentalissues and participates in the assessment of safety and effectiveness ofdental and orthopaedic devices.

Dr. Bernie Teitelbaum has 30 years of experience in the dental industry ina wide variety of management and executive functions ranging fromfinance and operations to sales and marketing. For the past 9 years hehas served as the Executive Director of the Dental Industry Association ofCanada (DIAC) and is responsible for moving the Association’s programsforward from conception to completion. Bernie is involved in all of theAssociation’s programs including membership, regulatory affairs, marketresearch, business and employee development, and conventions. Prior tojoining DIAC, Bernie held the positions of Director of Sales and Market-ing., Sales Manager of the Laboratory Products , and Treasurer and Con-troller of Dentsply Canada Ltd.

Dr. Paul Santerre is Associate Dean of Research at the Faculty of Dentistry,University of Toronto. He is a Full Professor, Dentistry, Division of Biologi-cal and Diagnostic Sciences, Full Professor, Institute for Biomaterials andBiomedical Engineering, University of Toronto and has cross appoint-ments in Chemical Engineering and Applied Chemistry as well as Metal-lurgy and Materials Science at the University of Toronto. CHARLIE FARQUHARSON is a national institution; his wife sometimes thinks he should be in

one. He was conceived on an Ontario farm in 1942 (his creator, Don Harron was getting his

grade 8 the easy way, as a member of the Farm Service Force working for six months at the

princely sum of twenty dollars per month.) But Charlie wasn't born until 1952. It happened

on a stage during a performance of the annual Canadian topical revue "Spring Thaw". It was

a five minute delivery, and it started a cottage industry which has produced ten best-selling

books, not to mention eighteen years on the television series "Hee Haw" which Charlie calls

"Sesame Street for Grown-ups". Among the tomes Charlie has turned out a Histry of Cana-

da 1972, a Jogfree of Canada, the World and Other Places, 1974, a KORN Farmer's Allmynack,

1976, a Parry Sound version of the Bible, Olde Charlie Farguharson's Testament, 1978, a hys-

terical review of the years 1972-1982 entitled Yer Last Decadent. Charlie was a contributor

to a book by his rich city cousin Valerie Rosedale in 1984 called DeBunk's History of the

Canadian Establishment. In 1986, Charlie almost gave up on agriculture with his book Cum

Buy the Farm. But the book is full of agricultural lore including advice on composting. "If yuh

takes from the soil yuh gotta giv back. Works fine with plants and animals. No waist there.

The wife and I has His and Hers piles of rotten old stuff. Hers is called composst and con-

tanes anythin' that'll give life to her gardin by rottin itself to deth. Tee bags, old lettusses, car-

rit tops, sawed dust, straw, twigs, hedgy clippins, plus domestical garbitch. It all gits togeth-

er and cooks over a long, long time. Yuh can't buy stuff like that, ya has to make it yerself,

and practickly ennybuddy kin do it hoo has jist had lunch."

Charlie has just completed his eleventh book entitled Charlie's A Broad. This is a travel book,

not a medical text. Published by MacMillan of Canada, it is in bookstores everywhere.

Gilbert Chapnick, D.D.S., F.I.C.D.This past year has seen an update and revision in the mediation process by the Ontario Dental Association.This has been done to comply with the new Privacy Laws and the liability concerns for the Ontario DentalAssociation and the members of the Mediation Committees. The relationship between the O.D.A.and theToronto Academy remains the same and the Academy assists the four Toronto Dental Societies in all medi-ation procedures.

There has been an increase in the number of requests for assistance from patients to resolve problems thathave developed with their dentist. Misunderstandings as to fees quoted for crowns and other restorativedentistry when there is a laboratory fee involved can create problems. It is mandatory that the laboratoryfee and the professional fee be quoted separately. Patients should be informed that following root canaltreatment, restorative treatment is required and there will be a separate fee for this service. Cosmetic proce-dures and lack of informed consent before proceeding with extensive restorative treatment have also made

resolutions difficult for Mediation Committees to resolve. Problems with patients do occur and should be resolved immediately ratherthan waiting and hoping that they will go away.

We are in the midst of a changing profession with new dental techniques, materials and patients who are more informed. There are manyethical issues focusing on dentistry that we as professionals must be aware of. This 70th Annual Winter Clinic has several excellent pro-grammes which will be discussing many of these important issues.

Service Award Recipient

Eva YoungEva’s introduction to Dentistry came 35 years ago, when she left her part time job as ateacher to become the office manager for Dr’s Don Coburn and Larry Levin in Hamilton,Ontario. Here she also received her first introduction to the dental industry by becomingan observer and occasional participant with Don, Larry, Doug Young and Wilson Southamin their activities at the Group at Cox in Stoney Creek.

A few years later, knowing that The Dental World was where she wanted to spend her future,Eva interviewed for and was offered a dental sales position by Les Jones, Canadian Managerof the Amalgamated Dental Division of Dentsply. Overjoyed, Eva literally jumped at the offer,and became the first female sales representative in the Canadian Dental Industry.

At Amalgamated, Eva used her dental experience to help graduating Ontario Dentistsorganize instrument and surgical tray setups for their new practices, and it was not long

afterward that her success in sales translated into a promotion to National Sales Manager for the company.

In the years since then, Eva has become a familiar face in the dental profession, where she has worked for 4 of our leadingdental manufacturers: as Group Product Manager for Caulk Dentsply; Canadian Managing Director for Kerr Sybron; World-wide Marketing Director for SciCan; and for the past 9 years, the Canadian General Manager for Ivoclar Vivadent.

Eva is a past President and Vice President of the Dental Industry Association of Canada and served on the DIAC Board ofDirectors for 12 years, holding portfolios in Regulatory Affairs, Professional Relations, and Communications. In 1995, as Pres-ident of DIAC, Eva initiated the development of the annual DIAC Survey of Canadian Dentistry, a comprehensive documentwhich has served to become a valuable strategic tool for DIAC members, the industry and the profession.

Eva was a member of Health Canada’s ISO 210 Working Group, served as a director on several Health Canada Stakeholder’sCommittees and on the International Congress of Dental Biomaterials where she helped host the Congress’s first Canadianmeeting in Toronto. Eva served as Vice Chair of the Dentistry Canada Fund and was an active member of their Board of Direc-tors for 8 years. She was a Founding Member and Chair of the DCF Business Advisory Council until 2004.

The Canadian Dental Association awarded Evawith their Special Friend of Canadian DentistryAward in 2002 and in 2004 she received the DCFOutstanding Service Award to Industry.

Locally, Eva was a longstanding participant of theOntario Dental Association’s Spring Meeting Com-mittee and has served on several working commit-tees with the Toronto Academy of Dentistrythrough the years. Dentistry, as prominent in herlife as it has been, takes a back seat to Eva’s great-est joy, her family; and her eyes sparkle at everyopportunity to talk – or boast - about her motherMartha, her husband Roger and their 4 childrenand 6 grandchildren.

Recently, Eva’s love of family motivated her toenter a new career – semi-retirement – so thatshe will have more time to enjoy them all - hope-fully for the next 35 years.

In recognition of his exceptional service to his profession and his community, characterizednot only by the breadth and quality of that involvement but also its duration, Dr. Jack Dalehas been chosen as the 2007 recipient of the Toronto Academy of Dentistry’s highest award– Honourary Life Membership.

Jack Dale received his college and dental education at the University of Toronto and hisorthodontic education at Harvard University, 1950- 1961.

He is currently the chairman of the board of The Charles H. Tweed International Foundationfor Orthodontic Research and Education, Tucson Arizona.

He has given in excess of 5702 lectures at the University of Toronto, at more than 105 other universities and hospitals, (378sessions when return visits are included), and to over 399 different associations and societies throughout the world, (678sessions when return visits are included) including 35 countries since 1959 on The Face, Growth and Development, EarlyTreatment including Serial Extraction and Edgewise Mechanotherapy, Excellence in Finishing, ABO, WBO, Long Term Sta-bility, Patient Motivation, Self Esteem, The Pursuit of Excellence, History and Biography. He has given in excess of 700 trib-utes.

He has been involved in over 75 publications including 23 books... with three in preparation and two in press. The booksrepresent a wide range of topics including graduate orthodontics education (Graber; Vanarsdall), basic science education(Ten Cate), public relations (AAO), standard of care (ABO), oral anatomy (University of Toronto), education, research andclinical care (Harvard University), treatment (Tweed Foundation), current affairs, (Tweed Foundation), and six on history,biography and recognition. Wherever possible, he has promoted the establishment of a World Board of Orthodontics,(Roberto Lima, Brazil).

He has served as president of over 15 organizations, including The Canadian Association of Orthodontists, The CanadianFoundation for the Advancement of Orthodontics (co-founder), The Ontario Association Orthodontists, The AmericanBoard of Orthodontics, The Harvard Society for the Advancement of Orthodontics (co-founder), The Charles H. TweedInternational Foundation for Orthodontic Research and Education, The International Association of Dental Research,Ontario Section, The Edward H. Angle Society of Orthodontics, Eastern Component and The American College of Dentists,Ontario Section. As a dental student he was president of his class for three years and president of the student council inhis fourth year.

As an undergraduate dental student, he was the recipient of over 40 scholarships and awards, and he has been honouredmany times since graduation including... the highest awards given by The American Board of Orthodontics, The AmericanAssociation of Orthodontists, The Canadian Association of Orthodontists, The Tweed Foundation, The Harvard Society forthe Advancement of Orthodontics, The Edward H. Angle Society of Orthodontics, Eastern Component, The OrthodonticEducation and Research Foundation, and The Faculty of Dentistry, University of Toronto. He has been recognized worldwide with The First World Award of Excellence at The First World Symposium on Orthodontics, San Diego, 2003. Dr. Dalereceived an American Board of Orthodontics Special Recognition Award, Orlando, 2004. His primary goal has been “the pursuit of excellence” and the highest possible “standard of care” for his patients and fororthodontic patients throughout the world. In his opinion, the most significant contribution that he has made is to theself esteem of his patients and to the education of his students.

Dr. Jack Dale has been married to Dr Anne Carlyle Dale since 1952. They have two daughters, Anne Forsyth Dale, a highschool mathematics and physics teacher and Dr. Hali Carlyle Dale, orthodontist, in practice with her father.
