Inappropriate shocks
Reduction of Inappropriate Shocks Associated with Dual Zone Programming
• 38 patients experienced shock due to non-VT/VF event
o 15 patients (4.6%) – SVT> Discrimination zone
o 24 patients (7.4%) – Oversensing
• No patients experienced a shock due to discrimination error in Conditional Shock (dual) zone
• Study demonstrated that dual zone programming is important tool for reducing inappropriate shock rate

Conclusioni • Il sistema S-ICD è sicuro e ben tollerato ed efficace nel trattamento delle
aritmie ventricolari. • La miniaturizzazione dei componenti del PM ha permesso lo sviluppo di
PM senza fili. • In atto questi nuovi devices sono limitati ai pazienti che necessitano una
stimolazione monocamerale in ventricolo destro. È probabile che una comunicazione wireless tra più devices impiantati all’interno del cuore può aprire ulteriori prospettive per l’elettrostimolazione.
• In ogni caso sono necessari ulteriori studi ed evidenze riguardo la sicurezza, la performance a lungo termine e la estraibilità di questi devices in caso di infezione.
Questioni aperte • Riescono questi leadless PM a rimanere performanti in un
lungo periodo di osservazione? • Possono essere estratti e come devono essere trattati i
pazienti che vanno incontro ad una infezione? • In caso di scarica della batteria è possibile lasciare il device
ed impiantarne un altro considerate le ridotte dimensioni? • Possono esserci delle complicanze trombo-emboliche nel
tempo? • Quale è il rapporto costo-beneficio?
