Subject: Additional information requested concerning USGS-FISC application for termination of License No. 09-25420-01.

Dear Mr. Lawyer:,

Below is the additional information you requested in your letter dated September 5,2006.

1. a) Timeline of decommissioning of the USGS-FISC Facility is attached. I have also attached a copy of the manifest for the last radioactive waste removal from the facility. Records of burials are non-applicable to this facility.

b) Records of offsite releases and waste disposal including effluent releases, release to sewers, incineration, and radioactive material spills are non-applicable to this facility.

2. A copy of the final status surveys of Room 15 and equipment therein and the waste shed are attached to this letter. The C14 vial that was stored at this facility was stored in its original container which was inside of a secondary containment tray that was housed in a tertiary containment tray. The C14 vial was never opened during the time it was housed at this facility. The secondary and tertiary containments were never breached and the entire containment package was transferred to UF on March 22, 2006.

3. Iodinel25. Date of last receipt was 11/22/05. Quantity N = 3, activity < 10 uCi per bottle. Quantity N = 3, activity < 9 uCi per bottle. Date of last use for all 12/8/05. I've attached a copy of the packing slip for this last order.

b) The USGS - FISC facility is a USGS owned building sitting on 28 acres of land owned by the University of Florida. The total area of the building is 28,000 square feet. The laboratory (Room 15) used for all radiation work is 450 square feet. The waste storage shed is 64 square feet.

c) This USGS science center is a facility consisting of office, bench and wet laboratory space. The primary focus of this center is aquatic research.

d) The whole property is 600 total acres of which this facility sits on 28 acres. The property is owned by the University of Florida. Our facility is surrounded by land used by the fisheries department of the University of Florida. However, the perimeter of the total grounds is surrounded by residential property.

e) All activities pertaining to the above license were bench top procedures. All procedures were basic RIA techniques. All activities pertaining to the license were performed in one laboratory in the facility (Room 15).

Timeline for Termination of License # 09-25420-01 for USGS-FISC

Dr. Timothy Gross no longer works for this USGS facility so the procedures for decommission and termination of License 09-25420-0 1 was begun on March 20,2006.

Isotopes were transferred from this USGS facility to the University of Florida on March 22,2006. Transfer paperwork is attached to this plan.

All waste was removed from Room 15 (RIA lab) and placed in the outside waste shed on March 24,2006.

Initial surveys were conducted in Room 15 to assess the status of the laboratory and equipment March 23Td and March 24* 2006.

A survey was conducted on two pieces of equipment (Gamma Counter and Centrifhge) before they were allowed to leave the Facility. A record of those surveys is on file. These pieces of equipment were transferred to UF on March 22,2006.

The NRC and USGS were informed of Dr. Gross' departure and guidance for the proper procedures was solicited. This e-mail dated April 12,2006.

An amendment letter dated April 19,2006 was sent to the NRC requesting amendments to the license to reflect the changes that have occurred and that this Facility will be terminating this license once the Facility has been decommissioned and the waste removed.

Final laboratory, ofice, hallway and equipment surveys were conducted between April 17& and April 27th. These results will be kept on file a minimum of three years after termination of the license. I will send a final swipe survey to Darrell Lyles at the USGS facility in Denver to verify my results. Sent e-mail to Mr. Lyles on September 6, 2006 letting him know I was ready to send my verification swipes to him. I have not heard back from him as of September 19, 2006.

Liquid Scintillation Counter was transferred to UF on May 4,2006. A record of the survey performed on the LSC before transfer is on file.

I placed a call to our local waste removal company (Perma-Fix of Florida). Left a phone message for Earl Fisher on May 19* to have him come to OUT facility to give me an estimate of cost to remove all waste.

We received our license amendment on May 24,2006.

Earl Fisher visited the facility the week of June 5" to set up a quote for removal of the waste stream.

Received quote from Perma-Fix on 6/26/06.

Put in request for fbnds 6/27/06.

Contract awarded to Perma-Fix of Florida on 7/19/06.

Waste removed on 8/9/06.

Letter sent to NRC requesting termination of license # 09-25420-01 sent on August 11, 2006.

All decommissioning surveys are on file at USGS. These surveys will be maintained for a minimum of three years. Any and all copies of these surveys will be provided upon request.
