Major Excursion Consent and Medical Information Form – Off

Campus Excursions Can be done as a ‘once a year’ form at the

beginning of the year. Must be done for all Swimming and Water Safety

programs and any activity involving swimming or the possibility of submersion (sailing, canoeing, rafting, etc.)

Date of first activity and last activity are included Must attach a separate itinerary/calendar of

events/activities that parents can keep. Dates can be approximate.

Campus Excursions Must advertise/remind parents of the upcoming

activities in the week(s) prior to the actual date Must ask parents to advise the school of any

changes to personal information/emergency contacts

It’s about parents making ‘informed’ consent Risk Management Plans are an integral part of

the process and must be signed off by Principals

Consent Form for Aquatic (Water-based) Activities

Must be done for all Swimming and Water Safety programs and any activity involving swimming or the possibility of submersion (sailing, canoeing, rafting etc)

Can be done with the Major Excursion Consent and Medical Information form, once a year.

Must have all activities listed with date and venue. Some schools have a reference to using alternative venues if a change becomes necessary.
