Submissions for this Award are to be typed & mailed/faxed/emailed (see relevant details below) with the heading “Award for Excellence” Application - Dementia & Recreation National Conference 2014. Submissions close strictly on Friday August 1 2014. Submissions must include contact details of the applicant including name, phone, email, organisation and address. The presentation of the Award will occur on the first day of the Conference & includes a plaque to commemorate the winner’s achievement. This $1,500 award is presented to recognise and reward innovation & excellence in Leisure, Recreation & Lifestyle service delivery in residential, community, home or acute care settings.
