3. Evidence-Practice Gap

Practice-gap Describe the evidence regarding the extent and nature of the problem or issue in dementia care that the project is seeking to address. Use super-script numbering to include references as end-notes in Section 17.

Best-practice Describe the evidence showing that the desired outcomes of the project constitute best-practice dementia care. If there is limited evidence in the specific context or setting of this project, describe other relevant evidence.

4. Barrier Analysis

A barrier analysis is a way of identifying the key barriers to changes in behaviour or practice. Barriers to change often exist at multiple levels (e.g., consumers/carers, care workers, health or medical professionals, management of institutions and organisations, and within high level regulatory or other systems). A multi-level barrier analysis can help both to direct and target interventions, and to determine their likely impact. For example, interventions to address barriers to practice change among care workers are unlikely to succeed or be sustained if related higher level barriers such as insufficient management support or inconsistent policies are not addressed.

With reference to the evidence-practice gap(s) that your project addresses, identify and discuss the most important barriers to best-practice dementia care at all relevant levels. Specifically identify which barriers are within and outside the scope of the project.

5. Strategies and Outcomes

Describe in detail the strategies or interventions that will be used to address each of the barriers identified as within the scope of the project in Section 4 and outline the anticipated outcomes and their expected timeframes (short, medium and long term). Refer to supporting evidence for the effectiveness of the strategies/interventions where applicable.

8. Consumer Involvement

In order to be eligible for funding the project must have a plan for meaningful involvement of consumers (people with dementia and their families and carers) in all stages of the project. This involvement should go beyond participation of a consumer on an advisory committee. Describe the extent of consumer involvement in the project and how this involvement will be achieved.

9. Evaluation

Describe how the project will be evaluated. This evaluation must be compatible with the evaluation and reporting framework developed by the program evaluators (attached). Provide details of the planned methodology (including instruments/indicators), data sources, analysis and reporting.

13. Detailed Budget

The maximum funding available through this program is $250,000 (excl GST). Provide a detailed itemised budget including non-Alzheimer’s Australia sources of funding and in-kind support. For external funding please indicate whether the funding has already been confirmed (and if so, provide a letter of offer or other relevant evidence). Identify and include any costs associated with consumer involvement in the project. Please indicate how much funding is being allocated to each partner organisation. Organisational overhead/administrative fees should be specifically identified and kept to a minimum.

14. Project Plan/Timeline

Provide a detailed timeline for the project including major project milestones and deliverables as well as a summary of the tasks which must be completed to achieve these milestones. All projects must be completed by the end of 2013 (note that this has been extended from the deadline of mid-2013 specified in the call for EOIs)
