Original old and new Passport of the student along with the residency visa page. Passport needs to be valid for 6 months from the date of return. Residency visa needs to be valid for at least 3 months from the date of return. Note: old passport is required if the child has a travel history

2 photographs 5X5 cm size white background (not older than 6 months, full face and ear visible) NOC letter from the parents addressed to Consulate General of the United States of America Passport copies with Visa page of the Father and the Mother (Clear and Colored copy) Salary certificate of the person who is the sponsor for the residence visa of the student. The salary

certificate must be addressed to the Consulate General of United States of America. (Those who own their company kindly submit the Trade License copy and 2 blank letter heads

with seal and signature of the owner or print the attached format in the company letter head then put stamp)

Last 6 months original bank statement of the sponsor with bank seal. Minimum Balance AED 15,000

Dear Parents, Those parents who would like to send their children on this trip are requested to fill in the tear off slip below and send it back with your children on or before 15TH January 2017 with an advance payment of AED 2000/- only along with a passport copy and residency visa copy of the student. Kindly note that teacher(s) will accompany the students as well as a representative tour manager.
