I, Applicant Andre Murray, of the City of Fredericton, County of York, in t

1. I Andr Murray as above indicated am the Applicant in this matter as suhave personal knowledge of the matters herein deposed to except where

otherwise stated.

2. My ASUS G73 SW computer, experienced a technical malfunction, hardrive failure, a severe enough failure/malfunction that I cannot use this subj

computer for recovery of my substantive material intended for submission t

Court of Queens Bench until it is repaired. This repair and possible datarecovery will require sending the computer, which is under warrenty, away

out of the country of Canada, for repair under warranty.

3. Through the use of various rough drafts and saved copies retrieved fromother sources I was able to reassemble and or recompose a large portion of

4. Compounding the issues is my disabling medical predicament anddeteriorating health.

5. I Andr Murray have been experiencing ill health for some time. I havebeen experiencing anxiety and depression. My symptoms include feelings sas: anxious, empty, hopeless, worried, helpless, worthless, guilty, irritable,

restless and a constant sense of foreboding doom.

6. I have lost interest in activities that once were pleasurable;7. I have experience loss of appetite, and I am losing weight.8. I have problems concentrating, remembering details, or making decision9. I am having difficulty communicating with others.10.I have been experiencing Insomnia and alternatively excessive sleepingfatigue, loss of energy, aches, pains and digestive problems that are resistantreatment.

11.I Andr Murray July 17, 2012, because of health and other concerns, didGrantor, appoint a Power of Attorney to oversee my legal affairs, if I becom

too ill to continue.

12.I Andr Murray have been seeing my doctor and been referred to expertmedical doctors for some time, regarding my deteriorating mental health is

irritability, hostility, exaggerated feeling of well-being, restlessness, or inab

to sit still; seizures; severe or persistent anxiety or trouble sleeping; severe

persistent headache or dizziness; shortness of breath; stomach pain; suicidathoughts or attempts; tremor; unusual or severe mental or mood changes;

unusual weakness; vision changes; worsening of depression.
