EnhanceFitness – What We Do All Data Collection for non-Y Affiliates (includes AAAs, community orgs, senior residences) Attendance Sheets

• Enter attendance for appropriate term

• Generate new attendance sheet for the next month, send to coordinators/instructors

EF Participant Information Forms

◦ Enter Participant Information, including contact and demographic info, and chronic conditions

Fitness Checks

◦ Includes fitness and health info, # of chair stands, # of arm curls, and # of seconds for each circuit of 8-foot up-and-go

Using TeleForm Forms Processing Software

◦ Optical Recognition Software ◦ Recognizes marks, characters and bar codes

Scanning Process

1. Scan Forms (Participant Information Forms and Evaluation Forms)

2. TeleForm reads bar codes, evaluates forms, flags items that need review/correction

3. Reviewer makes needed corrections

4. Data is committed to designated database

TeleForm Designer – Creating Forms

“Check All That Apply” Each condition has a yes/no answer and its own field in the database. The value in the database is either 1 (yes) or 0 (no).

“Choose Only One” This type of question allows for only one answer. Each choice is assigned a Value (Less than high school = 1, Some high school = 2).
