He gave a history of intermittent low grade fever, which was not associated with any history of chills and or rigor, cough with expectoration

or urinary symptoms. He noticed swelling of both the feet about 3months back which is gradually increasing. He gave a history of loss of appetite. There was no history of any urinary complaints like burning, blood in urine or voiding difficulty or nocturia. There was no history of facial puffiness, chest pain, joint swelling or pain or skin rash. He is a chronic smoker (15 pack years), chronic alcohol consumer. He gave a history of snake bite about 5 years back for which he was admitted and treated with dialysis, from which he had recovered completely. There was no history of any diabetes mellitus or tuberculosis in the past.

4 30.04.2016 A 30 year old young man working as a computer operator presented initially to neurology outpatient clinic with a history of headache mainly in the bitemporal region from the past one year. The headache was insidious in onset, gradually progressive, initially once in a month and now has increased to two episodes per day over the past 3 to 6 months and is associated with photopsia and photophobia at the peak of headache, headache increases on reading books, watching TV and on operating computer. He also gave history of slippage of objects from the hands, slippage of foot wear from both the feet and difficulty in getting up from the squatting position during the episodes of headache. The headache is relived with pain medications. There was no history of fever, loss of consciousness, vomiting, seizures, giddiness, diploplia, nystagmus or any aura. There was no history of any

trauma, ear or nasal discharge. There were no history of paraesthesia, cold or heat intolerance, wash basin attacks, difficulty to walk in dark surroundings and narrow paths. He gave history of reduced appetite from the past 6 months and difficulty to swallow solid foods. He was not a known diabetic, hypertensive or there was no past history of TB, asthma, epilepsy or heart disease.

complaints of head ache and anuria of one day duration.

6 25.06.2016 A 49 year old, man presented with chief complaints of abdominal distension, yellowish discolouration of eyes and fever of one month duration. He also complained of loss of appetite, early satiety and loss of weight. The abdominal distension was insidious in onset, associated with mild pain and fever with chills and rigor. The yellowish discolouration of eyes was insidious in onset, not associated with any fluctuation and remained of same intensity. There was no history of hematemesis, hemoptyssis, and nausea or vomiting. He said the colour of the stools was normal most of the times except for occasional dark coloured stools.

Patient did not give any history of tremors, palpitation, episodes of sweating or episodes of muscle weakness. There was no history of either excess weight gain or difficulty in climbing the stairs. He was evaluated outside with several investigations, prescribed some medicines and referred to SVIMS for further management. There was no prior history of hypertension, CVA, CKD or diabetes.

“Gender Harassment Committee” as specified in Section 4(1) of Sexual Harassment of Women at workplace (Prevention, Prohibition and Redressal) Act, 2013, was constituted, in our University, with the following members:

If no complaints are received in a month, a nil report will be furnished to the Director cum Vice Chancellor, SVIMS.
