Since our inception in 2000, Common Table Health Alliance has continued the legacy of addressing health and health care improvement in the region. We have strengthened the narrative in payment innovation, obesity prevention, public reporting, health equity, consumer activation, primary care quality, health and wellness and food insecurity. Some of our strongest contributions to the region include the many firsts we have achieved over CTHA’s 15 year history.

It is my honor to present the 2015 annual report for Common Table Health Alliance. As of December 31, 2015 I have retired as the first CEO of the organization. This provides a new window to frame the next chapter toward building long term capacity and sustainability. In

2015 the efforts of CTHA continue to focus on our long term strength in convening, translating information and operating community programs and partnerships.

In this report, we celebrate our top ten accomplishments on pages 4 and 5. We are very proud to be the administrative home of the newly formed Memphis Breast Cancer Consortium, funded by the Avon foundation (#1). This is continuation of the Sister Pact and Live Summit designed to address the health disparities in African American women in the Memphis Region with higher than desired mortality rates. In the past year we also landed a unique partnership with Proctor & Gamble (#7) to create a adult fitness zone in Audubon Park serving over 30,000 people. Finally, we pushed toward a new chapter by creating a regional alliance and partnership with Uplift Solutions (#9) to expand food financing across eight south eastern states.

The next chapter for CTHA is exciting and evolving. The chapter is one of great promise and commitment to continue the impact of the organization on the lives of all we touch. We thank the board, advisory committee, steering committees, staff, interns and other volunteers who make the work of CTHA exciting, fun and very meaningful. The next chapter is in motion, and we look forward to the role each of you continue to play in the telling of our story. It has been a privilege to serve as the CEO for 7 years!

The past two years as Chair of the CTHA Board have gone very quickly! As I prepare to turn over the title, honor and deeply-felt responsibility accompanying the role, you will find but a few of the many activities and accomplishments from both the past year and the past 15 years detailed in this report. We are not resting on past laurels however, rather, we use them to remind us of what is possible and where our next steps need to be focused.

Mine is not the only transition. Our beloved CEO, Renee’ Frazier, has accepted the title of “CEO Emeritus.” She leaves us in good shape, still searching out new possibilities, mentoring our staff and upcoming leaders and always bringing her enthusiasm and commitment for the work of Common Table Health Alliance. I also am excited to welcome the upcoming leadership

of Reggie Crenshaw as the next Board Chair. Reggie has long been engaged in a wide variety of CTHA activities. He is eager and well qualified to assume the responsibilities for the next phase of our work on a more direct basis.

We prepare for new leadership, while relying on Renee’s legacy and the inspiration to keep us moving forward. We also continue to need the commitment of our many community partners to support our initiatives and literally sustain us with your generous support. CTHA is truly a community resource and we look forward to continuing to serve and build alongside those who understand where our community has been and where it can go!

OUR 15-YEAR JOURNEY REPRESENTS A STORY OF FIRSTS

Since our inception in 2000, CTHA has had an obligation to the Greater Memphis area to be leaders in health innovation. We have rewritten our community’s health narrative to include unconventional collaborations, engaging conversations and new methods for combating health inequity. Our greatest distinction has been our ability to cut across silos to achieve consensus on the most significant health

barriers using our first-of-its-kind, multi-stakeholder convening model.

2010: CTHA conducted the first Patient Experience survey and published performance rankings of local physicians via its Health Care

Quality Matters website and mobile app.

2011: CTHA published the first regional Take Charge report on

Health Equity that examines how we live, work and play affects the

kind of care we receive.

2015: CTHA’s process improvement team effectively coached

Complete Health Care Center to receive the

first Level 3 NCQA Patient-Centered

Medical Home recognition in the Memphis region.

2006: CTHA hosted the first Shaping America’s

Youth Initiative, an unprecedented national

effort to address the childhood obesity and

inactivity crisis.

2014: CTHA launched the first national partnership with Procter & Gamble’s

wellness brand, Meta, to serve as inspiration

to communities around the country to start their journey to better health.

INSPIRING OUR NEIGHBORS TO ACHIEVE A LIFETIME OF HEALTHY HABITS

CTHA’s “Meta Memphians” inspired over 50,000 residents to make strides toward better health and wellness by

sharing their wellness journeys.

ENCOURAGING MEN TO KNOW THEIR NUMBERS

The Healthy Shelby Partnership’s 140/90:

Living Life Under Pressure campaign screened 2,000

community blood pressures over a six-month

period.

ALIGNING PARTNERS TO ADDRESS DISPARITY Twenty-nine member organizations of the Memphis Breast Cancer

Consortium built multi-stakeholder engagement by aiming toincrease the number of black women receiving screening

mammograms and treatment in targeted zip codes by 5 percent.

COMMUNICATING THE IMPORTANCE OF HEALTH LITERACYCTHA’s Health Literacy Education and Training Series received

over 60 survey responses from three hospital systems and one primary care clinic as part of its health literacy pilot program.

COMBATING CHRONIC DISEASE The Mayor’s Commitment to Fitness: 3rd Annual Million Calorie

Burn recruited 450 participants representing nine nonprofit organizations to fight chronic disease through movement and

calorie-burning.

REDUCING CHILDHOOD OBESITY

$32,000 donated from Mayor Luttrell’s Third Annual 5K to support

Healthy Shelby’s childhood obesity initiatives.

MAKING THE HEALTHY CHOICE THE EASY CHOICECTHA’s Preferred Choice program signed up 14

restaurants, caterers and hospital systems to adopt menu labeling, smaller portion sizes and menu options

that are 500 calories or less.

CREATING PUBLIC-PRIVATE PARTNERSHIPSCTHA’s partnership with Procter & Gamble’s wellness brand Meta and the City of Memphis created an Adult Fitness Zone

in Audubon Park for 30,000 visitors to take small steps to improve their health.

IMPROVING THE PATIENT EXPERIENCECTHA’s Process Improvement Team coached Complete Health Care

Center through the process to receive the first Level 3 NCQA Patient Centered Medical Home (PCMH) recognition in the Memphis region.

FACILITATING REGIONAL COLLABORATIONSCTHA and the Southeastern Health Equity Council have joined forces with Uplift Solutions to promote and support the development of a

Fresh Food Financing Fund across eight Southern states.

CTHA is a good steward of the dollars we raise to benefit the common good now and for generations to come. We aspire

for all children and families to have access to better, smarter food options – like the kids at the Covington Pike Head Start

Center who participate in daily physical activities and receive nutritious meals and snacks every day.

CTHA has devoted its financial resources and human capital to

programs and initiatives that bring about meaningful, lasting change. We illustrate our investment in the

community by taking leadership roles in wellness and fitness

initiatives like the Adult Fitness Zone in Audubon Park, where

Jacqueline Sales feels the project is a perfect addition to her workout.
