30-Day Readmission Rate

Hospitals/LTACH• NUMERATOR = Among patients in the

denominator, # readmitted within 30 days for any cause

• DEMOMINATOR = # of patients discharged alive during the time period, excluding transfers.

30-Day Readmission Rate

LTCF• NUMERATOR = Among patients in the

denominator, – # discharged to a hospital within 30 days of the

previous hospitalization – discharge for any cause.

• DEMOMINATOR =– # of patients discharged alive during the time

period AND – also were discharged from a hospital within 30

days prior.
