COVID-19 Vaccination CertificationThis guide demonstrates how to record a vaccine certification accommodation for COVID-19 in the SuccessFactors system.

Access the ERC, and then select Employee Self Service1

Begin the accommodation request process by selecting No for the question “Are you fully vaccinated?” 6

Important: Complete the form from top to bottom. The drop-down choices will change and pre-populate based on your selections, so it is important to do them in order. If you need to make changes later, start from the top again. Note: The second box is intentionally blank.

Complete the form by declining the vaccine and selecting the type of accommodation you are seeking (medical or religious). Select Save.7

Select Actions from the top right of the screen. Select Generate Document from the drop-down. 8

Select the document matching the type of accommodation you are seeking (medical or religious). Select either Send Directly by Email or Download. 9

Note:Send Directly to Email will only send to your Norfolk Southern email account.

Complete the accommodation form (medical or religious) 10

Return to the Vaccination Certification and select the Edit icon11

Upload your completed accommodation form by selecting Upload and then selecting the form from your device12

Certify the upload, verify all details, and select Save. You have completed submission of your accommodation request.13

An additional support document can be added along with the accommodation form by following the same instructions for the additional document location.
