We invite your application to this post which is within the acute children’s service based in Forth Valley Royal Hospital.

The Combined child health department in Forth Valley combines community and acute paediatrics. We currently have a team of 11 consultant paediatricians (general paediatrics, one of whom for community child health), 2 associate specialists, 4 specialty doctors in community child health and one in acute paediatrics, 4-7 specialty trainees, foundation year 2 and GP specialist trainees, and 2 advanced neonatal nurse practitioner (ANNP). The department provides comprehensive paediatric and neonatal care to a population of 300,000 people with 60,000 children under the age of 16 years and 3,200 deliveries per annum. There is a 25 bed combined medical and surgical Paediatric ward; a 20 bed Neonatal Unit (Level2) supporting a consultant led obstetric unit and an 8 bed clinical assessment and ambulatory care unit.

Forth Valley paediatric department participates in regional and national managed clinical networks allowing the development of specialist interests for all senior medical staff and ensuring children are cared for locally. The services are supported by a team of Community Children’s Nurses with general and specialist network links.

This appointment aims to support the delivery of our hospital-based children’s medical services.. Acute posts include a share of on call and out of hours working. All paediatric consultants are expected to have a minimum of Level 3 Child Protection training and participate in child protection examinations.

NHS Forth Valley is the largest employer in the area, with approximately 8000 staff from a wide range of professional and support occupations. Currently, services are provided in one acute hospital, four community hospitals and fifty-six health centres. Acute Services are based in purpose built 860 bedded Forth Valley Royal Hospital, which opened in 2011 and provides inpatient and outpatient services for a population of approximately 310,000.

THE FORTH VALLEY AREA

The working environment is excellent, as is the quality of life that Forth Valley has to offer them and their families. The area has stunning scenery, such as the Trossachs and Loch Lomond. It is steeped in history with Callander House in Falkirk, Stirling Castle, Bannockburn and the Wallace Monument to name but a few of the sites in the area. It also has many cultural and leisure facilities such as the Falkirk Wheel (the largest canal barge wheel in the world), the MacRobert Arts Centre at Stirling and the National Sports Academy at Stirling University.

Forth Valley also lies equidistant between Scotland’s two largest cities; Edinburgh and Glasgow. With their respective airports, both are little over half an hour from most parts of Forth Valley, on the Falkirk, Larbert and Stirling area lines.

FURTHER DEPARTMENTAL INFORMATION

The successful candidate will work within the team of 11 consultants who collectively provide comprehensive health care for children across community child health, acute general paediatrics, care of the newborn (including BAPM Level 2 neonatal intensive care).

Individual consultants provide specialist paediatric services in areas of subspecialty expertise or interest. Consultant paediatricians also take responsibility for coordination of different components of the combined child health service, including functioning as a link to managed care networks.

The consultant paediatrician will share the workload with the other career grade staff in the department. He or she will share leadership in other areas to complement the existing staff in post. Acute consultant paediatricians also share in the acute paediatric on-call rota, as well as sharing the acute daytime cover in general paediatrics and/or neonatology.

The consultants, associate specialists, and specialty doctor are supported by training grade paediatric doctors in hospital and community services.

Departmental Information

Acute Paediatrics

Hospital-based medical paediatric services are provided on a single inpatient site at Forth Valley Royal Hospital. This includes: general paediatrics, ambulatory and day case investigations unit, neonatal intensive and special care baby unit, paediatric surgery from age 3 years, and anaesthetics (general surgery, urology, ENT, ophthalmology, orthopaedics and dental), paediatric accident and emergency service, and a number of radiography services for children (including plain X-ray, contrast studies, Isotope scans, ultrasound, CT and non-sedated MRI).

Inpatient General Paediatrics

Approximately 5000 children are admitted every year. As well as standard inpatient beds there are also facilities for close observation and stabilisation for transfer to PICU in Glasgow or Edinburgh.

The average length of stay for our admissions is less than 1.1 days. Consultant paediatricians share a regular rota to provide continuous support and supervision to the inpatient children’s ward, neonatal unit, and ambulatory unit during normal working hours Monday to Friday. This is achieved by having a consultant of the week. This same group of consultant paediatricians and one associate specialist also share the on call rota for out-of-hours cover for the paediatric service at nights and weekends. We use the inpatient service as the basis of clinical teaching to senior medical students from Edinburgh University who attend during the academic year in blocks of time.

Ambulatory Paediatric Care

Around 2000 children per year are seen in the ambulatory care unit or are admitted as day-cases. The service is in a separate area adjacent to the children’s ward.

Care of the Newborn

Over 3000 babies are born in Forth Valley Royal Hospital each year. All consultant paediatricians and one associate specialist share the weekly rota to provide continuous support to the antenatal services, labour ward, neonatal intensive care unit and the postnatal wards. The

consultant paediatricians also share an out-of-hours nights and weekends on-call rota and will provide the regular daytime weekend cover to the neonatal and ambulatory care units.

Neonatal care in Forth Valley is provided at BAPM Level 2 and within the West of Scotland Neonatal Managed Clinical Network with shared clinical guidelines, audit and education activities. Planning for at risk deliveries or postnatal need for ongoing intensive care is coordinated through the Perinatal Advisory Service which is currently regional but may extend to a national service in the future.

Outpatient Clinics

Around 1700 new medical paediatric patients and between 5000 and 6000 return patients are assessed every year at the outpatient clinics in Forth Valley Royal Hospital, Stirling Community Hospital and Clackmannanshire Community Hospital. Consultant Paediatricians provide general outpatient clinics and specialist clinics in neurology, respiratory, endocrine, diabetes, cystic fibrosis, gastroenterology, and neuro-development. Many of these are linked to regional or national Managed Care Networks. Tertiary clinics are also provided locally by visiting consultants from Glasgow (cardiology, nephrology, rheumatology, clinical genetics, cleft lip and palate). Cystic fibrosis care is shared with the Lothian CF team as part of a national MCN. We also have specialist clinics in enuresis and encopresis.

Community Children’s Nursing

A community children’s nursing team is attached to the department providing out of hospital care to children with chronic disease and life limiting conditions including complex care.

NHS Forth Valley has embraced the principles of GIRFEC (Getting it Right for Every Child) and the Early Years Collaborative that supports this approach to coordinating and integrating the services that provide for infants’ and children’s needs.

Community Child Health

The department covers a population of 60,000 children under the age of 16 years. The population of Forth Valley lives mainly in the city of Stirling, several large towns (Falkirk, Alloa, Grangemouth, Bo'ness), and the more rural areas of Stirlingshire and Clackmannanshire. The living standards in Forth Valley are high with small areas of severe deprivation. The region has diverse ethnic backgrounds, but with no established ethnic communities.

The department of community child health works closely with the education and social services in the three local authorities. The department of community child health operates within the combined child health service and offers the following services under supervision and co-ordination of consultant paediatricians.

Disability & Additional Needs

Services for children with disabilities are provided from within multi-disciplinary teams that operated at different localities. No formal Child Development Centres are operational yet, but plans have been discussed.

Services are also provided for neonatal follow up of babies with increased risk for disabilities and also for adolescents in transition to adulthood. Early assessment teams are in place for multi-disciplinary approach to children with suspected developmental problems. The department also works closely with National Children's Homes locally and the two children’s hospices in provision of services to children in respite care.

Specialist Clinics

Services include children with constipation, encopresis, enuresis, behavioural problems, autism, attention deficit hyperactivity disorder, hearing impairment, visual impairment, and sexual health.

Child Protection

All consultant paediatricians provide early assessment and examination of children, supported by nurse advisors. One consultant paediatrician is in post as Lead for Child Protection and takes responsibilities for compiling agreed protocols and guidelines. This consultant represents the department in the managed clinical network for west of Scotland. The paediatricians have a close working relationship with forensic medical examiner (FME) in Forth Valley. The FME’s have experience in child sex abuse (CSA) examination working with the paediatric consultants who have undertaken additional training in CSA assessment. When required, colposcopy examinations are performed on site in the paediatric unit.

Child Health Screening

The department carries out electronic recording of child health screening data, and continues to undergo developments in line with national recommendations.

Health Promotion and Disease Prevention

The department is involved in the primary immunisation programme by setting local guidelines and advice to general practitioners and health visitors. The department works closely with the Health Promotion team at NHS Forth Valley and the Local Authority's Community Schools.

Adoption and Fostering

One paediatrician is designated medical advisor for each of the Local Authorities’ Resource panels and provides assessment and reports on accommodated children. The department also provides health and medical input to “looked after children” in local authorities' accommodation and also for the adoption and fostering services. A nurse-led service for looked after children has been developed and co-ordinates services with partners in the local authorities. It has set up an effective system for information exchange, monitoring, evaluation and follow-up, based on the established practice of health assessments for individual children.

Relationship with other departments

CAMH services are based within the Mental Health Unit though there is no inpatient paediatric psychiatric ward on site. There are 3 consultant child psychiatrists providing an area service, who also occasionally utilise in-patient paediatric beds in conjunction with the paediatric consultants. Separate paediatric clinical psychology staff are also based the hospital, working in close association with child psychiatry. There is a close link with regional educational psychology services.

There is a close relationship with the Royal Hospital for Children, Glasgow, our principal tertiary referral centre for childhood haematology/oncology, neonatal surgery, cardiology, PICU and other supra-regional services. Close working and networking with this tertiary centre and with RHSC Edinburgh is encouraged.

Teaching and Training and research

NHS Forth Valley places a high priority on training and development across all staff groups and strongly supports the continuing medical education programme for Consultants. The postgraduate centre is located within the main hospital building and is well-equipped.

Three specialty trainees are attached to the department from the East of Scotland deanery for core paediatric training. The department runs a weekly teaching programme. The West of Scotland deanery provides General Practice Registrars and Foundation Year 2 trainees in paediatrics for training at tier 1 in paediatrics. Individual teaching in preparation for DCH and MRCPCH examination is offered to all trainees. Three GPs with special interest in paediatrics have been attached to the Forth Valley Department as the NES GP Paediatric Fellowship. Forth Valley has supported the training of midwives in examination of the newborn and has established the NES Scottish Maternity training course in Resuscitation of the Newborn. In June 2014 the first APLS course in Forth Valley Royal Hospital took place. The Scottish Centre for Simulation and Clinical Human Factors, based at Forth Valley Royal Hospital, is collaborating with our department in developing additional training opportunities in acute paediatric and neonatal clinical management.

Training of senior neonatal nurses as advanced neonatal nurse practitioners is underway. A similar programme for training existing paediatric nurses as advanced paediatric nurse practitioners is also in place. New posts have been created and filled with these nurses.

Undergraduate Students from the University of Edinburgh are attached regularly for their child health block and students from University of Dundee attend for special study modules.

Staff are encouraged to participate in research relevant to their work both as lead investigator or contributing to multi-centre research and to support colleagues who are so engaged. Support for training and participation in research and Quality Improvement is offered by NHS Forth Valley

LOCUM CONSULTANT GENERAL PAEDIATRICIAN

As agreed with the General Manager, Associate Medical Director (Women, Children and Sexual Health Directorate), and along with other colleagues, to provide an area wide service in General Paediatrics with responsibility for the prevention, diagnosis and management of illness, and for the proper functioning of the service.

Out–of-hours responsibilities for Forth Valley Royal Hospital shared with the other Consultants as part of a Consultant team;

To participate in leading as well as the provision of Paediatric Services in Forth Valley, including the supervision and training of medical staff, nurses, and allied health professionals. This will include leadership of the neurodisability service.

Within agreed areas of competence, to provide cover for consultant colleagues during annual and study leave or at such other times as agreed with the General Manager and Associate Medical Director (Women, Children and Sexual Health Directorate).

Offer professional supervision, training and appraisal of junior medical, nursing and technical staff as appropriate so as to enhance role extension and job skills within the Paediatric service in order to improve the service provided to patients.

Motivate staff within the service, through leading by example, and fostering good working relationships at all levels in line with the principles of the local partnership agreements.

Contribute to developments and initiatives within the organisation as appropriate and as requested.

Undertake teaching, accreditation and examination duties, and contribute to postgraduate and continuing medical education activity across the area of service.

Agree and implement an annual medical audit and clinical governance programme with the Associate Medical Director (Women, Children and Sexual Health Directorate), in line with Service’s Clinical Effectiveness Strategy.

Participate fully in consultant appraisal and personal development planning activities.

Ensure the efficient and effective use of the Board’s resources.

Work with Paediatric colleagues to ensure the effective and efficient development and implementation of support systems.

Support the Paediatric Department’s Clinical Governance agenda.

Operate within the scope of agreed policies in Women, Children and Sexual Health Directorate .

The requirements and responsibilities of the post will change through time in line with clinical advances and developments in services provided by the NHS in Forth Valley.

CONTRACTUAL DETAILSContracted activities for this appointment will be defined and agreed with the post holder, the Associate Medical Director and the General Manager.

The post holders will be professionally responsible to the Associate Medical Director and managerially responsible to the General Manager, Women, Children and Sexual Health Directorate.

The Consultants in the department will agree arrangements for cover of duties.

This post is available on a full time basis but the Board welcomes applications from individuals who are unable to undertake a full time commitment and may wish to work on either a part-time or job share basis.

This post is offered on a fixed term basis for 6 months

The terms and conditions of service applicable to this appointment are the Consultant grade terms and conditions of service as amended from time to time.

NHS Forth Valley will require each Consultant to agree with the General Manager and Associate Medical Director or Clinical Director a job plan of fixed and other commitments which will be reviewed on an annual basis and revised as necessary to meet the requirements of the local and national service issues.

Consultants will be required to participate in annual appraisal and to fulfil the requirements for revalidation.

Formal appointment to this post will be conditional upon satisfactory completion of all pre-employment checks, including satisfactory medical screening.

NHS Forth Valley does not negotiate salary placements. On commencement of employment, salary will be determined in accordance with Section 5 of the terms and conditions of service.

This appointment will be superannuable under the NHS Superannuation Scheme, which is contracted out of the State Earnings Related Pension Scheme (SERPS). The post holder is required to agree a detailed job plan which will be reviewed at least annually.   Efforts will be made to complete job planning prior to the date of commencement in post, or, if this is not possible, as soon as possible following this date. 

 The detail of fixed commitments, including any special interest, will be finalised following appointment of the successful applicant. This is a 10 Programmed Activity post of which 1 SPA will be allocated for Personal Development Following commencement in post, depending on the needs of the service and the particular attributes of the post holder, it may be possible to negotiate additional time for Supporting Professional Activities to include teaching and training. This will require the approval of the General Manager, Associate Medical Director and Clinical Director. Supporting professional activities will include all of the Consultant’s professional duties and commitment. The exact timing and location of the Supporting Professional Activities will be agreed during the job plan review with the General Manager, Associate Medical Director and Clinical Director and included in the prospective job plan.

The Consultant job plan will be flexible but broadly on a ten week cycle. Acute service cover and/or on call, if applicable, is approximately one in ten. The acute inpatient service operates a “Consultant of the Week” system.

Criminal Convictions - Protecting Vulnerable Groups (PVG) Scheme

The Protecting Vulnerable Groups (PVG) scheme has replaced the previous disclosure arrangements for people who work with vulnerable groups. The duties of this post require the post holder to undertake regulated work with children and young people (individuals aged under 18 years) and / or protected adults (individuals aged 16 or over who is provided with (and thus receives) a type of care, support or welfare service).

The post holder will be required to be a member of the PVG Scheme and to consent to NHS Forth Valley obtaining a Scheme Record or Scheme Record Update, as appropriate. Any failure to disclose convictions could result in the withdrawal of an offer of employment.

Occupational Health Screening

This post involves the performance of exposure prone procedures and therefore the successful applicant will be required to produce evidence of their Hepatitis B and C Immunity Status prior to appointment and to undergo satisfactory health screening. Any offer of employment will be subject to satisfactory Occupational Health assessment.

Confidentiality

During the course of their employment with NHS Forth Valley, employees may have access to information about staff or patients or other aspects of NHS Forth Valley’s activities, about which they have a duty to maintain confidentiality at all times. In common with all other staff, the post holder will have, in addition, a responsibility to ensure that information relating to his or her work and the operation of the Forth Valley Acute Services in general is kept and maintained securely in accordance with the requirements of the Data Protection Act (1998), the Board’s policies and good practice. In particular, the disclosure of commercial or other confidential information which may affect the Board’s business interests or endanger the survival of any of its services will be regarded as a fundamental breach of the mutual confidence which must exist between the employer and the employee.

Criteria Essential DesirableQualifications Membership of the Royal College of

Paediatrics and Child Health or a qualification of an equivalent standard

Full GMC registration and inclusion on the specialist register with a licence to practise.

Certificate of Completion of Specialist Training (CCT) / Certificate of Eligibility for Specialist Registration (CESR).

Applicants who are within 6 months of achieving CCT / CESR, at the date of interview are eligible to apply.

Higher degree e.g. MSc, MD or PhD higher degree

MBA or other relevant diplomas

Respiratory or Neonatal experience would be an advantage

Child Protection certificate/experience

Knowledge and Skills Willing to work unsupervised and make decisions

Good written and oral communication skills

Demonstrated ability to manage time effectively

Experience of working in a multi-disciplinary team Demonstrated ability to work unsupervised and make decisions

High level of competence in the clinical aspects of General Paediatrics and evidence of an ability to develop new skills and train colleagues as appropriate.

Good information technology and administrative skills.

Demonstrated ability to work unsupervised and make decisions

Demonstrated ability to lead a multi-disciplinary team

Demonstrated ability to communicate effectively e.g. report writing; thesis

Academic Participation in research or clinical audit to improve clinical care.

Participation in departmental teaching

Formal research training; demonstrated ability to design a research or audit project

Formal training in teaching methods; demonstrated ability in teaching / training.

Management Understanding of management issues including medical discipline; clinical governance, service planning and willingness to contribute to the activity of the NHS in Forth Valley.

Demonstrated ability to manage e.g. project work; mentoring junior staff
