Definition

- inflammatory eye reaction- Usually unilateral - Redness of limbus- Sterile infiltrative keratitis

Page 3: Contact Lens-Induced Acute Red Eye(CLARE)

Symptoms:

- Redness - Irritation - Mild pain- Photophobia- Watering

Page 4: Contact Lens-Induced Acute Red Eye(CLARE)

sign- Infiltration

- Anterior chamber involvement (-)

- Fluorescein staining (-)

Page 5: Contact Lens-Induced Acute Red Eye(CLARE)

Etiology

- Bacterial contamination- Hypoxia - Tight lens- Deposit

Page 6: Contact Lens-Induced Acute Red Eye(CLARE)

Deposit

- Denatured protein ,calcium ,lipid and…- Mechanical trauma

Page 7: Contact Lens-Induced Acute Red Eye(CLARE)

Bacterial contamination

- Gram negative- Gram positive

Page 8: Contact Lens-Induced Acute Red Eye(CLARE)

Corneal Hypoxia

Page 9: Contact Lens-Induced Acute Red Eye(CLARE)

Examination

- Slit lamp eye examination:- Low mag. Diffuse illumination- High mag. Direct and indirect illumination- Fluorescein staining method

Page 10: Contact Lens-Induced Acute Red Eye(CLARE)

infiltrations

-sterile infiltration

-infectious infiltration

Page 11: Contact Lens-Induced Acute Red Eye(CLARE)
Page 12: Contact Lens-Induced Acute Red Eye(CLARE)

Sterile infiltration

- Contact lens induced peripheral ulcer(CLPU)- Contact lens induced acute red eye(CLARE)- Infiltrative keratitis(IK)

Page 13: Contact Lens-Induced Acute Red Eye(CLARE)

Differential diagnosis

-sterile infiltrations:- CLARE- CLPUC- Infiltrative keratitis(IK)- microbial keratitis(MK)

Page 14: Contact Lens-Induced Acute Red Eye(CLARE)

CLPU

- Unilateral inflammatory reaction- Noticed in morning- Epithelial loss- Anterior chamber involvement- Corneal staining- Watering ,limbal redness ,mild to severe pain ,foreign

body sensation .

Page 15: Contact Lens-Induced Acute Red Eye(CLARE)

Infiltrative keratitis

- Sub epithelial infiltration- Daily occurrence - Burning ,mild pain ,redness- Unilateral inflammatory reaction

Page 16: Contact Lens-Induced Acute Red Eye(CLARE)
Page 17: Contact Lens-Induced Acute Red Eye(CLARE)

Microbial keratitis- Corneal staining more than 2mm in diameter- Focal infiltration more than 1.5mm in diameter- Severe ocular pain that persist or worsens after lens removal- Extensive anterior chamber flare- Significant vision loss

prognosis

- Discomfort- Photophobia- Infiltration- Vision loss

Page 20: Contact Lens-Induced Acute Red Eye(CLARE)

Management

- Cease lens usage - Improve hygiene- Fit frequent replacement lens- Fit low water content lens- Avoid tight lens fit
