larger absolute recurrence risks

absolute risk reductions with RT than
did older women and those with low-
grade tumors…
… radiotherapy leads to significant clinical
benefits, with an overall 16% absolute
decrease (19% vs 35%) in the risk of
breast cancer recurrence and a 4%
absolute decrease (21% vs 25%, p<0.0001)
in the risk of dying from breast cancer.

With respect to the first issue, findings from
the EBCTCG meta-analysis clearly show that
patients with breast cancer have clinically
relevant rates of persistent locoregional
disease after conservation surgery with and
without systemic treatments. The magnitude
of this risk is dependent on several factors,
including axillary lymph-node status, tumour
size, tumour grade, age, and oestrogen-
receptor (ER) status. The analysis shows that
radiation is highly successful in eradication
of much of this disease.
… the success of radiation in reduction
of
recurrence varies according to biological
subtype of breast cancer. In the EBCTCG
study, the proportional reduction in risk
of recurrence with radiation was nearly
60% for ER-positive disease treated with
tamoxifen, but only 35% for ER-negative
disease.
The data from many thousands of clinical trial
patients reviewed by the EBCTCG
investigators continue to provide us with
crucially important insights. The data
reinforce the important role that
radiotherapy has in management of breast
cancer, and the fact that the benefits of
radiation are complementary to the advances
in both surgery and systemic treatment is
particularly rewarding. The incremental
benefits of each component of treatment
contribute to the ongoing success in
reduction of breast cancer mortality rates.
Le bénéfice de la RT a également
été démontré chez les patientes
traitées par mastectomie pN +
(pN1-3 et pN >3), avec un gain
absolu en contrôle local de 15% à
5 ans avec et en parallèle une
augmentation absolue de la
survie spécifique de 4% à 15 ans
(ratio 4:1)

Comme tous les traitements, la RT peut entraîner certaines toxicités, en particulier au niveau cardiaque et/ou pulmonaire.Toutefois, depuis une vingtaine d’années, de très importants progrès ont été réalisés:
amélioration technique des appareils(accélérateurs avec collimateurs
multilames/électrons)
apport considérable de l’imagerie (TDM++)
informatisation optimisée permettant des calculs très complexes avec des dosimétries
adaptées respectant les « organes critiques »

Afin de réduire la durée des traitements et
de diminuer les délais entre chirurgie et
radiothérapie dus à l’encombrement des
services, de nouvelles modalités
d’irradiation se sont développées depuis
une quinzaine d’années, en particulier pour
les patientes âgées (>70 ans) qui
représentent désormais ≈ 25% de la
population traitée

Il s’agit de :
- l’irradiation hypofractionnée (après chirurgie conservatrice ou
mastectomie)- l’irradiation partielle du sein (après chirurgie conservatrice pour
des indications très sélectionnées)

Dépistage
Le dépistage mammographique a changé le « profil » clinico-biologique des cancers du sein.
En parallèle, le vieillissement de la population s’accroît régulièrement.
Le traitement conservateur peut-être appliqué à environ 75% des patientes.
Cette nouvelle méta-analyse confirme que la radiothérapie mammaire a un rôle fondamental dans la réduction du risque de RL.
CONCLUSIONS (I)
Les facteurs de risque de RL ont été
« affinés » avec l’introduction des
« profils moléculaires », en complément
des facteurs de risque classiques comme
l’âge, le grade, l’état des marges.

L’identification de patientes à faible risque de RL pouvant bénéficier de modalités d’irradiation particulières (IHF/IPS)
