The National Awards are open to everyone and is one of the ways in which we recognise and appreciate our colleagues who consistently deliver outstanding performance.

The Awards recognise individuals, teams and sites who demonstrate excellence, a commitment to The Regis Way (Optimism, Passion, Integrity and Respect) and make Regis a better place to live and to work.

Each nomination is judged on merit and for all those meeting the Criteria, a personalised certificate is presented at a ceremony.

The finalists are those who have gone above and beyond their expected daily tasks – people who improve the daily lives of those they care for and work with. Finalists and their partners will be flown to Melbourne to hear the announcement of the winners and runners up at the Awards Dinner Friday 25th November 2016.

We recognise staff who have made a positive impact to Regis by sharing their ideas that have improved or developed a product, service or process through:

• An initiative which has had a positive impact on our customers or the business

• An outcome which demonstrates Regis’ commitment to ongoing improvement and an innovative approach

• Proving the importance of this initiative or innovation.

We recognise individuals or teams who have provided outstanding and compassionate dementia care through:

• An outstanding contribution to improving care for dementia care recipients

• Positive results in care recipient dementia care everyday, sometimes in the face of adversity or other challenges

• Understanding, compassion or courage

• A willingness to ensure our dementia care recipients are the centre of everything they do.

10. OUTSTANDING SAFETY CONTRIBUTION

Open to teams or individuals

We recognise staff who have shown an outstanding commitment to injury prevention through:

• An active commitment to injury prevention

• Personal leadership around creating workplace routines or procedures which lead to a safer workplace and/or reduced injury rates

• Identifying and implementing effective solutions to control hazards in the workplace

• Actions which have improved health and safety outcomes for care recipients and their colleagues.

2. SERVICE EXCELLENCE (REGIS PEOPLE SUPPORTING REGIS PEOPLE)

6. OUTSTANDING CARER (CARE RECIPIENT CARE)

Open to carers in non-dementia care

(residential and other)

We recognise individuals or teams who have provided outstanding and compassionate care through:

• An outstanding contribution to improving care for care recipients

• Positive results in care recipient care everyday, sometimes in the face of adversity or other challenges

• Understanding, compassion or courage

• A willingness to ensure our care recipients are the centre of everything they do.

9. VOLUNTEER OF THE YEAROpen to volunteers

We recognise volunteers who have provided outstanding service through:

• Working tirelessly at a facility for an extended period

• Providing exceptional care for and support older people and their carers

• Adding to the everyday experiences of our care recipients and enriched their lives

• Providing great support to our staff.

• Setting and meeting high standards and helping others achieve them

• Demonstrating timely and effective decision making, reporting, and risk management skills

• Showing a commitment to safety, quality systems, social and environmental responsibility

• Encouraging creativity, collaboration, positivity and innovation

• Helping staff expand their skills, confidence, talent and thinking

• Giving their own time to advise and guide team members’ careers.

12. CEO AWARDCEO/MD only

The winner is selected from all 2016 shortlisted nominations by the CEO and goes to the team or individual whose actions make them a true ambassador of Regis. This award recognises high integrity, exceptional skills, leadership and/or innovation, and those who have had a positive impact on our customers and/or collegues.

CRITERIA Nominations may be made by: a current staff member, care recipient, family member, community stakeholder or a volunteer. No individual self-nominations accepted apart from the Facility of the Year category.

All individual staff nominees must have more than 12 months service at the aged care facility or service (including home care, day therapy, ILU or retirement village), or with Regis.

All projects or programs nominated must have been in full operational mode for a minimum of 6 months and a maximum of 24 months by 19th August 2016.

All nominations must address the award criteria and be submitted via mail on this nomination form (PO Box 8373, Armadale Vic 3143) or online (regisawards.com.au) by 5pm on 19th August 2016 Late nominations will not be accepted.

We encourage you to include supporting documentation (photos, examples, positive feedback etc), which address and provide further evidence of the award criteria being met.
