Incontinence is common and complex:Opportunities exist to improve resident outcomes and increase quality of life and care focus related to the management of incontinence. Many residents in skilled nursing facilities have some level of incontinence, which, if not thoroughly managed, may lead to negative outcomes, including unnecessary hospitalizations and heightened regulatory oversight.

Revised CMS regulations:CMS reviewed and revised the federal guidelines as well as survey guidance for incontinence management a few years ago (F315) and related F-Tags, with a heightened focus right now on:

• Readmissions to the hospital

• Facility protocols

• Response and implementation of incontinence management

Enhanced facility management: The CMS interpretive guidance for incontinence management indicates that a skilled nursing facility must ensure that they have updated policies, procedures, processes, education and monitoring activities around an individual’s right regarding the management of incontinence. This includes treatment options, risks/benefits, advanced care planning, as well as the person's individual advance directives related to the management of incontinence.

Skilled nursing facilities strive toward an environment in which every resident remains continent for as long as possible.

Effective incontinence management requires an ongoing facility-wide commitment to preventing incontinence-associated skin breakdown, providing resident comfort, identifying and addressing barriers to continence, and addressing any misconceptions that residents, families and staff may have about incontinence. Incontinence care

Using a measuring tape: 1 Measure the resident from hip to hip and multiply

by 2 and add 2 inches.

2 Once you have the total hip measurement, you will want to use the McKesson sizing chart and determine the proper size based on the product you are using.

OR

In order to help provide resident comfort and maximum absorbency, selecting the right size is important. You can determine the proper size by either using a measuring tape or using the residents’ height and weight.

McKesson provides complimentary sizing and application kits that contain:

- Measuring tape color coded by size - Sizing charts- Application video and written guide

Using the residents' height and weight: You will need to use the McKesson sizing chart. For example, if the resident is 5'5" and 140 pounds, then you would select a size Medium. See sizing charts on pages 15-16.

Hip bone to hip bone = 19" (49 cm)19" x 2 (49 cm x 2) = 38" (96.5 cm)

After the soiled product has been removed and the skin cleansed, assessed and protected, you are ready to apply the new brief, per facility policy.

Standing:

1 Completely unfold the new brief lengthwise, including the side panels.

2 Hold brief at each end and gently pull, activating the leg gathers to create a "cupped" shape for optimal fit and performance.

3 Place the product between the thighs from the front with tape tabs to the rear.

Lying down:

1 Completely unfold the new brief lengthwise, including the side panels.

2 Hold brief at each end and gently pull, activating the leg gathers to create a "cupped" shape for optimal fit and performance.

3 Roll resident to one side and spread out the brief, including side panels, underneath resident’s buttocks with tape tabs facing the head of the bed.

4 Roll the resident back over.

5 Pull the front of the product up into the perineal area. Center it front to back, making sure the product is symmetrically positioned on the body.

Application guide for briefs

Remaining application steps:

1 Spread the front side panels against the hips toward the back of the body.

2 Then overlap the rear side panels on top of these toward the front of the body. Use the hook and loop tabs to secure the brief. The tape tabs always go on the front of the body.

The bottom two tabs should be angled slightly upward. The top two tabs should point straight across, or be angled slightly downward.

3 Press down on the tabs to secure them to the outside.

4 Smooth out all wrinkles and folds and check the fit of the product. Make any adjustments as necessary.

Remember, the closure tabs can be unfastened and fastened again, if needed.

5 Gently pull the skin of each inner thigh downward and away from the perineal area. This allows the leg cuffs inside the brief to move into place.

Adapt these suggestions to meet the specific needs of your residents and the facility policies.
