Completing the GHQ Group Benefit Services, Inc. (GBS) utilizes the HERO System by Milliman to evaluate a group based on their personal health questionnaires. We ask employers to complete a Group Health Questionnaire (GHQ) as a way to get more information about the group as a whole.

Link Your consultant will email you two links from the HERO System. You will be responsible for sending the Personal Health Questionnaire (PHQ) to your employees for their completion. In the email that your consultant sends you, are instructions you send to your employee to give them a brief overview of why they are completing the PHQ. You will be responsible for completing the Group Health Questionnaire (GHQ).

Register Click on the link for the GHQ. The screen you come to says Register or Login. Since this is your first time entering information into the system, you will need to register for an account.

Welcome The first screen you get to after Register, is Welcome! This is where you set your username and password. It is a good idea to write down your username and password in the event that you can’t complete the entire questionnaire at one time and have to log back in.

Completing the GHQ The next series of screens are to show you the questions and information that you will be required to complete/answer. Only the questions marked with a red asterisk are required.

Group Health Questions The next section is asking questions on behalf of your company and if you are aware of any medical conditions, pregnancies or anyone on COBRA.

Sign and Submit At the end of the questionnaire is a Privacy Notification Policy. Please read it and make sure you understand how this form is used and that it does not bind coverage. You must click the Electronic Signature to complete the form.

After you click Submit, you will get another screen that says Thank you for completing the GHQ. If you didn’t get this far because you had to research a section or you just did not have time, you can use the original link that was sent to you by your consultant to log back in using the username and password that you created when you registered.
