2011 COMMUNITY WELLNESS ASSESSMENT REPORT ARBORFIELD

What is Primary Health Care?

Primary Health Care is a framework for providing everyday health care services. Health care professionals work proactively with intersectoral partners and community groups to address broader community needs.

Primary Health Care works towards:

• Preventing people from becoming ill or injured

• Managing chronic conditions

• Making the most effective use of health care provider expertise

• Treating acute and episodic illness

• Efficiency and co-ordination of services

• Accessing the right service at the right time

• Individuals playing an active role in their own health care

• Understanding that factors outside the health system influence individual and community health (Determinants of Health)

Introduction

A Primary Health Care Provider Team has been in existence in Carrot River/Arborfield/Zenon Park since 2001. The Team consists of health professionals and community representatives and meets on a regular basis to address wellness needs within the communities.

One way to identify wellness needs is through the use of a Community Wellness Assessment. The first community assessment in Arborfield was conducted on March 2008 utilizing a community forum to identify areas of concern, with 44 community members attending. In 2009, a different approach was taken to conduct the Assessment to connect with a wider cross section of the community. The team “went to the community to gather information” versus expecting the community to attend a public meeting. A booth was set up at the Arborfield Beeland Coop and community members were encouraged to voice their opinions on survey questions. In total 40 community members completed the survey questionnaire. The team shared the information gathered with local Mayor and Community Advisory Committee and developed action plans to address some of the identified needs. Needs that were beyond the Primary Health Care Team or local Advisory Committee’s ability to resolve were forwarded to the appropriate group/agency.

Within the community of Arborfield the Community Advisory Committee consists of community members, local health center administrator and nurse practitioner. This committee has been working collaboratively to address various local community issues for several years..

Primary Health Care Team and partners have accomplished the following since the last assessment:

• PARTY Program (Preventing Alcohol Related Trauma in Youth) has been offered to high school students in Arborfield and Zenon Park every second year.

• Primary Health Care Team continues to support the annual Tri-Unity Challenge partnership (community walk/run to promote physical activity). This will be the third year Primary Health Care has partnered with various agencies to support the walk/run.

• Depression Support Group

• Collaborated with Arborfield Advisory Committee and Arborfield High School to add CPR classes into curriculum

• Worked with Arborfield Advisory Committee to find meals on wheels coordinator for the community.

• Collaborated with Saskatchewan In-Motion for the development of an In-Motion park and walking trails signs in Arborfield

• Seniors Falls Prevention education offered in Arborfield and Zenon Park in 2009

• Developed directory of visiting health professionals services in English and French languages

• During months of Jan/Feb/March 2011 supported a “Fun Fitness Challenge” within the communities of Arborfield/Zenon Park and Carrot River. Zenon Park was the successful winner and received $6,000. The monies are designated to help build local walking or bike trails.

• Offered monthly grocery store bag stuffers and shelving labels depicting healthy food choices in communities of Arborfield and Zenon Park

• Organized a Healthy Cooking class for January/February which had to be cancelled due to lack of registrants

• Applied for and received a SGI ATV Safety grant to offer an ATV Safety Course to youth. Class was cancelled due to lack of registrants and grant monies were returned to SGI.

• February 2011 Team members and community representatives participated in Primary Health Care Strategic planning to look at ways to enhance or improve Primary Health Care services for Carrot River, Arborfield and Zenon Park

In 2011, members of the Primary Health Care Team and Arborfield Community Advisory Committee were present at the Arborfield Beeland June 16th 12- 4pm to conduct the Community Wellness Assessment. Community members shopping at the store were invited to complete the survey questionnaire. In total 65 surveys were completed. The Primary Health Care Provider Team extends a sincere thank you to the members of the Arborfield Advisory Committee for their assistance in conducting the 2011 Community Wellness Assessment.

New to the survey this year was the addition of questions 5. (e) and (f) – refer to attached survey questionnaire. The information gathered from these two specific questions will also be shared with Primary Health Care Program Awareness Team who are now investigating how community members are currently finding out about health information as well as how community members would prefer to find out about this information. This team will be making recommendations to health care teams on how best to create awareness of health information, programs and services within our KTHR communities.

Information received was categorized according to three Population Health Promotion Pillars:

• Accessible Nutritious Foods

• Active Living

• Mental Well-Being/Substance Use and Abuse

• and an additional category entitled “Other” (ideas beyond the scope of the Team)

Earlier in the month of June members of the community advisory committee had circulated the survey to various groups and individuals within the community. Also, the Primary Health Care Clinic staff had offered clients accessing services at the clinic an opportunity to complete the survey questionnaire

make your community a healthier and safer place to live, work, and play? - More NP and doctors closer - To keep a nurse practitioner in Arborfield x 4 - Provide or enable nursing outpatient treatments locally on weekends (home care, or health

centre – IV’s, dressings, etc) - Ensure the excellent service we get at Arborfield Health Centre remains – nurse practitioner,

lab, x-ray – consultation with various visiting therapies – foot care, dietitian, speech or therapist, etc.

- Have Arborfield Health Centre able to do dressings and other services on weekends instead of having to travel to Nipawin for a simple service.

- More being able to stay in your own community when you need care - Go to people’s houses to bandage them up and stuff - Have specialists closer - More support for seniors - More respite beds - Have someone (nurse practitioners) on call or at the care center available for nursing or

doctor services - Get us more clinic hours. - Have an RN or LPN on duty or call for people who have had surgery and need dressings,

injections and whatever else they may need - Keep our health care close to home and home care help - Bath by home care aid

- A need to have our health centre services continue as it is quite satisfactory now. There is a need for coverage for outpatient treatments for weekends. I feel home care is very beneficial for an aging population such as we have in our community. With home care we can stay in our homes longer.

- Keep more health care closer to home and home care help - More workers available for Home Care and EMT and more days for our Nurse Practitioner - Increase mental health councilors/staff - Healthy food choices at school canteen - Promote healthier diets - Bring in more fresh fruit trucks (maybe roadside sales) - Put information on town face book site to keep our town clean and not throw glass bottles
