How Can YOUR organization become a Community Partner?
1.) Visit the HHSC website at: http://www.hhsc.state.tx.us/hhsc_projects/cpp/
2.) Fill out an interest form online at:https://docs.google.com/spreadsheet/viewform?formkey=dHhsR0lhSzV6RXJQeGFoZUtXR3luY3c6MA#gid=0
3.) Decide whether your organization is best fit to become a Level 1 or Level 2Community Partner
4.) Print out the Memorandum of Understanding (MOU) and go over it with yourorganization. You can find the MOU online at:
http://www.hhsc.state.tx.us/hhsc_projects/cpp/docs/cpp-mou.pdf
5.) Once HHSC has approved your interest form submit your MOU to the state.(At thispoint volunteers must complete online navigator training if you choose to become alevel 2 Community Partner.)
6.) HHSC will contact you once your organization been officially approved
