Back-log Coding

There are times when due to staff sickness, holidays, resignations etc the CodingDepartment can be overwhelmed with a back-log of work. With the introduction of Paymentby Results it is now even more critical thatepisodes of care are coded in a timely fashion.

In these instances 3M’s ACC and experiencedcoding staff can provide temporary codingassistance to reduce your back-log. This workcan be undertaken on site or remotely. Our staffwill work wherever the coding is carried out(e.g. coding offices, wards, departments) andcan cover all specialties.

Many NHS Trusts have already used ourservices and have considerably reduced theircoding back-log without interrupting theworkflow of their own coders.

Clinical Coding Audit Service

3M Training and Coding ACC personnel areexperienced in providing an on-site clinicalcoding audit service within small and large-scalecoding functions throughout the UnitedKingdom. Staff have undertaken and passedthe NHS Information Authority Advanced AuditCourse and are NHSIA Approved Auditors.

The clinical coding audit service is available toNHS Trusts throughout the UK. A typical auditpackage will include a pre-meeting, audit, exitmeeting, followed by a detailed report.

Pre-Clinical Coding Audit MeetingThis meeting is used to discuss objectives ofthe audit, local issues, source documentation, data items to be audited and any comparativeanalysis required.

Clinical Coding AuditThe clinical coding audit is carried out from thesource documentation (e.g. case notes)normally available to the coding department. The clinical coding audit will also assess thequality of source documents. The auditor willtake local coding guidelines and clinical termprotocols into consideration.

Exit MeetingThis may be arranged for immediate feedbackfollowing the clinical coding audit.

Clinical Coding Audit ReportThe clinical coding audit report will provideoverall results, which follow NHSIA coding AuditGuidelines.

3M can provide follow-up training to deal withany recommendations made in the Report. Allfeedback is presented in a constructive manner.

Further follow-up is also available to assist informulating a strategy for improvement of thequality of coding in relation to any areashighlighted by the audit that are not specificallyrelated to clinical coder training.

Healthcare Resource Group (HRG)Assurance

3M’s HRG Assurance programme is modelledafter 3M’s Diagnostic Related Groups (DRGs)Assurance in the US.

Documentation deficiencies in the US havebeen estimated to account for over $7.5 billionannually in lost hospital reimbursement forservices rendered. Of the documentation errorsthat result in lost reimbursement:

• 43% are documentation errors

• 31% from insufficient documentation

• 12% from lost documentation

• 40% are the result of providing medicallyunnecessary services

• 17% are from coding errors

3M’s DRG Assurance programme has beeninfluential in assisting US hospitals in improvingtheir clinician’s documentation processes sothat reimbursement loss is minimised.

HRG Assurance Objective• Accurate and complete case note

documentation

• Assessment of case note quality

• Education of healthcare personnel andclinicians

• Implementation of a concurrent reviewprogramme

• Obtaining accurate and complete case notedocumentation

• Evaluation and monitoring with frequentfeedback

Benefits Include• Optimal financial flows through the

appropriate HRG assignment

• Improved communication between coders, management and clinicians

• Improved quality of care, care planning, resource utilisation and outcome studies

• Accurate patient Severity-of-Illness (SOI) andRisk-of-Mortality (ROM) profiling for hospitalmanagement and the medical staff

• Compliance with Government rules andregulations

3M HRG Assurance Methodology

• Review random sample of case notes

• Perform coding audit or review recentexternal coding audit

• Reference Hospital’s draft health recordspolicy document

• Interview clinical coding management andcoding team to understand workflow andprocesses

• Interview nurses and clinicians to understandcurrent documentation process

• Train clinicians and coders on best practiceexamples found during assessment

• Provide period monitoring

• Reassess one year later to measure impact
