Summer 2011- Club Kindergarten -

This form is for campers at the Club Kindergarten site only. Please sign and return the permission slip to the ParkDistrict before our first trip. If we do not have a completed copy of this form, your child will not be able to attendcamp field trips. Children need to wear their camp T-shirts on all field trip days. Transportation will be by schoolbus; a flyer containing this and other important information will be given out each week. All field trips are subject tochange. Notification of changes will be given out weekly.
