Rob Franks, DO, FAOASM Welcome, spring. By now, most of you are in the thick of the spring sports season. The weather has finally started to change, and we are outside for lacrosse, baseball, softball, track and field, etc. Congratulations to you all for getting to the final sprint of the 2016-17 academic year. With the warmer weather, I am drawn to thoughts of Las Vegas. It won’t be long now until we meet at Harrah’s Las Vegas for the AOASM Spring Conference, May 3-6, and Ultrasound Pre-Conference Workshop, May 1-2, in fabulous Las Vegas, Nevada. Registration and hotel reservations are open and filling quickly. If you have not yet registered, please do so promptly, as Dr. John Dougherty has put together a solid academic program, which includes topics such as boxing, combat medicine, and concussion. With assistance from Brooke Miller, our meeting planner, the conference’s social events will allow you to mingle and reconnect with colleagues from across the country at the High Roller Ferris wheel, as well as at the Cinco de Mayo Party, Opening Reception, and Award of Fellow all at Harrah’s. There will be something for every member, both academically and socially, as AOASM settles onto the Vegas strip in a few weeks. When registering, please take a moment to update your personal information to assist the Membership Committee, so we have your most up-to-date contact information to keep you current on all news and information concerning the Academy. As we get into later spring, we hope to have the new website up and running. As this edition of Sidelines goes to print, our Website Committee should be putting the finishing touches on content for the new website and testing its interface for use by all Academy members. Thank you to all the committee members from not only the Website Committee but also several other committees, for adding to the content and suggesting interface for the new website to make it user-friendly for all Academy members. I would like to personally thank Alexandra Myers, who is heading to Germany for May’s Sportsosteopathy Meeting in Cologne, Germany, during our conference. I, and the rest of the Executive Committee and Board of Directors, appreciate your efforts in heading to Germany to represent our organization at the expense of attending the AOASM conference. Both I and your colleagues appreciate this gesture of goodwill to teach our colleagues in Germany. Finally, I would like to take a moment to say thank you to all members of the Academy. I cannot believe a year has gone so quickly. It has been an honor and a privilege to represent you as President of the AOASM for the last year. I feel like it was just yesterday that I had my first address in Tempe. This year has truly been a team effort, and I could not have done it without every member of the team. I would like to extend many thanks to and recognize the tireless efforts of: The Executive Committee and The Rees Group. We have spent a lot of time together, and I greatly appreciate all your time and willingness to quickly and succinctly master any task that needed to be done on behalf of the Academy. The Board of Directors. Thank you for giving your time and for being able to meet at a moment’s notice and quickly act all year on issues that needed expeditious and appropriate action to benefit the members of the Academy. The committee chairs and committee members. You are the backbone of the organization and have done amazing work on so many projects over the course of the year. Whether it was the design of the new website or the creation of student conferences, your tireless work has made this year a success. The many members of the Academy who stepped up and asked, “What can I do?” Thank you to the many members who carried out successfully whatever task was asked of them. The students, interns, residents, and fellows. You are the lifeblood of our organization. Thank you for your passion, dedication, and hard work on all the programs you participated in this year. We are a stronger organization for all you have done. I am confident in handing over the presidency of the Academy to Dr. Jeff Bytomski that AOASM is in great hands for the next year and the immediate future. It has been an absolute pleasure and honor to serve you over this last year, and I am excited to assist in my new role as Immediate Past President. I look forward to seeing you all soon in Las Vegas. She is a lifelong athlete, and ran Division I Track and Field and received her bachelors in Exercise Science. Join Janelle on Thursday, May 4, at 7:00 a.m., for a 45-minute workout with proper dynamic warm-up, exhilarating strength and conditioning exercises, partner drills and circuits, plus effective cool-down and recovery. Athletes and the Arts Written by: Rebeccah Rodriguez, DO “The stage is not merely the meeting place of all the arts, but is also the return of art to life.” Oscar Wilde. Performing Arts include forms of creative activity that are performed in front of an audience, such as drama, music, dance, and vocal. When vocalists of any caliber or level enter your office, be ready to preserve the vocal cords! Even though they might present as a basic URI or with allergic rhinitis or GERD, some of the standard medications might not be suitable treatment. This summary provides the sports medicine specialist some specialized education, vocal hygiene tips, and treatment facts in treating vocalists. Young school-age children are more susceptible to getting viral colds, hoarseness, sore throats, and laryngitis. Young boys have more voice changes with puberty. Yelling at sporting events or at social gatherings and improper singing, if done to excess or during a period of illness, all can cause damage to a youngster’s voice. Hoarseness is often the first sign of more than an upper respiratory tract infection; it can indicate a number of different vocal problems. Look out for chronic or recurrent hoarseness with Vocal Fold Nodules. They may cause loss of ability to sing high notes softly, frequent voice breaks, and vocal fatigue. This is common in male adolescents. Remember that vocalists are athletes, too, and put major time into their craft. The conditions governing these patients’ lives include early exposure to high expectations of excellence, incessant demands for perfection, long periods of intense practicing, fierce competition, and high levels of anxiety associated with performance. Vocal hygiene tips to encourage and use as treatment in your patients: 1. Drink 10-12 glasses of fluids a day, preferably without ice. The best beverage for singers is plain hot water with dashes of lemon and honey 2. Avoid alcohol and all forms of smoke. Limit spicy foods. 3. Avoid all forms of whispering, including loud “stage” whispers. 4. Avoid throat clearing, yelling, or gargling. The vibrations irritate the vocal cords. 5. Keep environmental temperatures comfortable. 6. Avoid drying medications (antihistamines, mint, menthol, liquid medication, decongestants). If they are necessary, increase intake of fluids. 7. Rest the voice completely for hoarseness, laryngitis, or pharyngitis. Take Tylenol® only. 8. While traveling—land or air—keep conversation to a minimum. 9. Avoid travel the day of performance, especially before difficult or repeated performances, so the body can rest. 10. See a doctor if throat problems last more than five days, especially if a fever develops, if there is a loss of appetite or fatigue, or if neck glands become swollen or tender. SAOASM Highlights Written by: Charlie Kenyon, SAOASM National Student Chair We are excited to announce that the 2017 national SAOASM meeting will take place during the afternoon of Thursday, May 4, during the AOASM Clinical Conference in Las Vegas, Nevada. This year’s program features multiple presentations by current fellows and will bring back a full slate of the ever-popular student IGNITE lectures. The IGNITE lectures are an incredible venue for our student members to practice public speaking skills during the high- energy, five-minute presentation. The meeting concludes with business items such as voting for the 2017-2018 SAOASM Executive Board. Many thanks to all those who came out for the 2017 regional conferences. We are very proud of our regional reps, who pulled off extremely successful meetings in all five student regions! Please read below for highlights from the Central and Midwest conferences. On behalf of the 2016-2017 Executive Board, we are excited to see everyone at the national conference, and continue to build friendships and gain mentors that will last throughout our future careers. Charlie, Stephen, Joseph, Jessica, Justin, Juliana, and Ashley 2017 Central Region SAOASM Conference Written by: Justin Stumph, 2016-2017 Central Regional Representative The 2017 Central Region SAOASM Conference was held at the OU-HCOM Dublin campus on March 25. A group of around 30 students started the day with a workshop on ankle injuries. Next, the group learned from Drs. Ben Bring and Ben Ahrens as they introduced us to the basics of musculoskeletal ultrasound. This was followed by a presentation on eye injuries in sports with AOASM’s Second Vice President, Dr. Shawn Kerger. Finally, Dr. Bring talked with the group about concussions. Photo Right: Students and attending physicians after a successful conference day! 2017 Midwest Region SAOASM Conference Written by: Jessica Hersh, 2016-2017 Midwest Regional Representative The SAOASM Midwest Regional Sports Medicine Conference was hosted at the University of North Texas Health Science Center-Texas College of Osteopathic Medicine on April 1, 2017, in Fort Worth, Texas. Tarrant County Medical Society supported the event with funding for accreditation, which attracted physicians and other medical professionals from a wide range of specialties to attend, creating a great networking environment for students, as well as an environment for active learning and engaging discussions. We had an exciting program of lectures and workshops for students and professionals. The conference featured a variety of disciplines including Orthopedics, Family Medicine, Psychology, Osteopathic Medicine, and a skills Sports Ultrasound workshop. These sessions offered an opportunity to learn more about all levels of athletes, knee to back injuries, young to old athletes, the neuropsychology of concussion, as well as gain hands-on training in osteopathic manipulation and fine-tuning ultrasound skills. Click here to read more! Teaching in China: Featuring Dr. Pat Leary In April 2017, Pat Leary, DO, FAOASM, went to Shenzhen, China to teach about shoulder injuries. Journal Article Spotlight: Clinical Journal of Sport Medicine Effectiveness of Shockwave Treatment Combined With Eccentric Training for Patellar Tendinopathy: A Double-Blinded Randomized Study Objective: To evaluate the effectiveness of a combined treatment of focused shockwave therapy (ESWT) and eccentric training compared with sham-shockwave therapy (placebo) and eccentric training in participants with patellar tendinopathy (PT) after 24 weeks. Design: Randomized controlled trial. Setting: Sports medicine departments of a university hospital and a general hospital in the Netherlands. Participants: Fifty-two physically active male and female participants with a clinical diagnosis of PT (mean age: 28.6 years; range, 18-45) were randomly allocated to the ESWT (n = 22) or sham shockwave (n = 30). Interventions: Extracorporeal shockwave therapy and sham shockwave were applied in 3 sessions at 1-week intervals with a piezoelectric device. All participants were instructed to perform eccentric exercises (3 sets of 15 repetitions twice a day) for 3 months on a decline board at home. Main Outcome Measures: The Victorian Institute of Sport Assessment-Patella (VISA-P) scores (primary), pain scores during functional knee loading tests, and Likert score (secondary) were registered at baseline and at 6, 12, and 24 weeks after the start with the ESWT or sham-shockwave treatment. Results: No significant differences for the primary and secondary outcome measures were found between the groups. In the ESWT/eccentric group, the VISA-P increased from 54.5 ± 15.4 to 70.9 ± 17.8, whereas the VISA-P in the sham-shockwave/eccentric group increased from 58.9 ± 14.6 to 78.2 ± 15.8 (between-group change in VISA-P at 24 weeks −4.8; 95% confidence interval, −12.7 to 3.0, P = 0.150). Conclusions: This study showed no additional effect of 3 sessions ESWT in participants with PT treated with eccentric exercises. The results should be interpreted with caution because of small sample size and considerable loss to follow-up, particularly in the ESWT group. Read more... Journal Article Spotlight: British Journal of Sports Medicine International criteria for electrocardiographic interpretation in athletes Sudden cardiac death (SCD) is the leading cause of mortality in athletes during sport. A variety of mostly hereditary, structural or electrical cardiac disorders are associated with SCD in young athletes, the majority of which can be identified or suggested by abnormalities on a resting 12-lead electrocardiogram (ECG). Whether used for diagnostic or screening purposes, physicians responsible for the cardiovascular care of athletes should be knowledgeable and competent in ECG interpretation in athletes. However, in most countries a shortage of physician expertise limits wider application of the ECG in the care of the athlete. A critical need exists for physician education in modern ECG interpretation that distinguishes normal physiological adaptations in athletes from distinctly abnormal findings suggestive of underlying pathology. Since the original 2010 European Society of Cardiology recommendations for ECG interpretation in athletes, ECG standards have evolved quickly, advanced by a growing body of scientific data and investigations that both examine proposed criteria sets and establish new evidence to guide refinements. On 26–27 February 2015, an international group of experts in sports cardiology, inherited cardiac disease, and sports medicine convened in Seattle, Washington (USA), to update contemporary standards for ECG interpretation in athletes. The objective of the meeting was to define and revise ECG interpretation standards based on new and emerging research and to develop a clear guide to the proper evaluation of ECG abnormalities in athletes. This statement represents an international consensus for ECG interpretation in athletes and provides expert opinion-based recommendations linking specific ECG abnormalities and the secondary evaluation for conditions associated with SCD. The2017CentralRegionSAOASMConferencewasheldattheOU-HCOMDublincampusonMarch25.Agroupofaround30studentsstartedthedaywithaworkshoponankleinjuries.Next,thegrouplearnedfromDrs.BenBringandBenAhrensastheyintroducedustothebasicsofmusculoskeletalultrasound.ThiswasfollowedbyapresentationoneyeinjuriesinsportswithAOASM’sSecondVicePresident,Dr.ShawnKerger.Finally,Dr.Bringtalkedwiththegroupaboutconcussions.Overall,itwasagreatdaytolearnmoreaboutosteopathicsportsmedicine.

EffectivenessofShockwaveTreatmentCombinedWithEccentricTrainingforPatellarTendinopathy:ADouble-BlindedRandomizedStudy

Objective:Toevaluatetheeffectivenessofacombinedtreatmentoffocusedshockwavetherapy(ESWT)andeccentrictrainingcomparedwithsham-shockwavetherapy(placebo)andeccentrictraininginparticipantswithpatellartendinopathy(PT)after24weeks.

Design:Randomizedcontrolledtrial.

Setting:SportsmedicinedepartmentsofauniversityhospitalandageneralhospitalintheNetherlands.

Participants:Fifty-twophysicallyactivemaleandfemaleparticipantswithaclinicaldiagnosisofPT(meanage:28.6years;range,18-45)wererandomlyallocatedtotheESWT(n=22)orshamshockwave(n=30).

Interventions:Extracorporealshockwavetherapyandshamshockwavewereappliedin3sessionsat1-weekintervalswithapiezoelectricdevice.Allparticipantswereinstructedtoperformeccentricexercises(3setsof15repetitionstwiceaday)for3monthsonadeclineboardathome.

MainOutcomeMeasures:TheVictorianInstituteofSportAssessment-Patella(VISA-P)scores(primary),painscoresduringfunctionalkneeloadingtests,andLikertscore(secondary)wereregisteredatbaselineandat6,12,and24weeksafterthestartwiththeESWTorsham-shockwavetreatment.

Results:Nosignificantdifferencesfortheprimaryandsecondaryoutcomemeasureswerefoundbetweenthegroups.IntheESWT/eccentricgroup,theVISA-Pincreasedfrom54.5±15.4to70.9±17.8,whereastheVISA-Pinthesham-shockwave/eccentricgroupincreasedfrom58.9±14.6to78.2±15.8(between-groupchangeinVISA-Pat24weeks−4.8;95%confidenceinterval,−12.7to3.0,P=0.150).

Conclusions:Thisstudyshowednoadditionaleffectof3sessionsESWTinparticipantswithPTtreatedwitheccentricexercises.Theresultsshouldbeinterpretedwithcautionbecauseofsmallsamplesizeandconsiderablelosstofollow-up,particularlyintheESWTgroup.Readmore...
