Clay County Community Health Survey Highlights and Trends January 2012 Acknowledgements The purpose of the Clay County Community Health Survey is to identify quality of life and community health issues from the perspective of Clay County residents. We collected 383 surveys between September 2011 and December 2011. The purpose of the Clay County Community Health Survey is to identify quality of life and community

health issues from the perspective of Clay County residents. We collected 383 surveys between

September 2011 and December 2011.
