All users must complete/sign a liabilityform or check in sheet. Staff reserves the

right to remove any member or guestthat exhibits behavior that is believed to

be unsafe or inappropriate.

Use of profanity is prohibited at all times,·Fighting and horseplay is prohibited

·No spitting, chewing tobacco, alcohol,smoking, or vaping in the facility.

Headphones must be worn to listen topersonal music..

-Outside food or coolers will not beallowed in the facility-No glass containers

-Water containers must have a securelid and cannot be taken onto the gym

floor

-Renting Equipment cost $3-Failure to return equipment or damaging

equipment could result in not beingpermitted back to the Civic Center or

charges to your Civic Plus account-Non-traditional gymnasium equipment

must be approved by the rec staff

-Spit, chew tobacco, drinkalcohol, smoke, or vape in the

facility-DO NOT Open front GYM Doors

for other people EVERYONEMUST sign in upstairs-Dunk or hang on rims

-NO organization team practices duringopen gym times

-NO organized group gatherings duringopen gym times where groups act in an

exclusive manner-NO pets are allowed with except of those

assisting a person with a disability.

·All users 14 years of age oryounger must beaccompanied by an adult(18+ years) at all times.

DRESS CODE·All users must wear appropriate

clothing at all times (shorts or sweats,shirts, and non-marking sneakers).

·Clean, scuff-resistant shoes must beworn in activity areas at all times

unless designated by the activity orprogram instructors.

-Staff reserves the right to add, amend ordelete rules as necessary.-Staff reserves the right to remove anymember or guest that exhibits behaviorthat is believed to be unsafe orinappropriate.-Staff reserves the right to resolve allconflicts during play on the gym floor.
