Dear Participant and Parent/Guardian,
Welcome to the City of San Dimas Parks and Recreation Student Union. We
hope that your participation in our program will be a rewarding experience
for you both. We feel strongly that open communication between
participant’s, parents/guardians and staff is essential to fully meeting the
needs of all of the participants.
This Registration Packet is intended to promote an understanding of our
program and its policies; as well as foster cooperation amongst our Student
Union families and staff. We hope to create mutual respect between
parents/guardians and staff, building a partnership for the benefit of each
participant.
Please take time to review the following registration packet as a family. It
is important that everyone is familiar with this information.

GENERAL INFORMATION
The Student Union offers a FREE drop-in afterschool program (Monday-Friday) for teens
grades 6-12 throughout the school year supervised by trained Recreation Staff. The
Student Union provides a variety of afterschool.

To be a safe, supportive and fun Student Union where teens can discover positive
experiences and build lasting friendships.

Hours of Operation
The Student Unions normal operating hours are Monday thru Friday; Monday, August 19,
2019 – Thursday, June 4, 2019 2:30 pm-6 pm except on minimum days, 1:30pm-6pm. Hours
may change due to early dismissal, and School Breaks. The Student Union observes national
holidays. (Parents should discuss the arrival and departure time with their teens and
strongly discourage them from leaving the facility prior to the agreed time.)

Sign In/Out Procedures and Membership ID Policy
At The Student Union, your student will be issued a Student Union Membership card when
registering. When your participant arrives at the facility/program, they must give their ID
card to a staff member who will scan them in for the day. It is mandatory for every
participant to bring their ID card each day they come to The Student Union. The
participant will also sign in and sign out on the sign in/out sheet daily. The participant is
responsible for keeping the card safe and bringing it to the program. If the participant
loses or damages their card, the parent/guardian will be responsible for purchasing a
replacement card for $1.
Facility Access
Students are not permitted to access amenities or areas past the second doors on both
the men’s and women’s entrances. This includes but is not limited to locker rooms,
restrooms, saunas, showers, racquetball courts, gym and fitness room. Exception includes
when Student Union staff is supervising/leading a program or activity in which participants
have registered for.
Fee
The Student Union Drop-in Program is free to all participants grades 6th to 12th.
Forms and Waivers
All participants must submit an Emergency Information & Waivers, Teen and Parent
Acknowledgement, Permission Slip for Teen Programs/Excursions and Special Events and
Student Union Liability Waiver,prior to participation. Forms may be found on pages of this
packet. Forms may be submitted on the first day of participation or emailed to
[email protected] or [email protected] .
In the event any of the information provided on the attached waiver changes please notify the
Student Union staff so participants’ files can be updated.
Allergy Awareness
Children with severe peanut and/or tree nut allergies may be at risk for exposure to
allergens through airborne or physical contact contamination. Due to potential life-
threatening allergies in our participants, we do NOT allow products that contain, or may
contain, peanuts and/or tree nuts in the Student Union program. Please DO NOT send your
child with any products that contain peanut or tree nut products, or products that include
similar statements on their labels:
• may contain peanuts and/or tree nuts
• made in a facility that processes peanuts and/or tree nuts
• made on shared equipment that also process peanuts and/or tree nuts
Transportation
It is the responsibility of the participant and parent to arrange transportation to and from
the facility.
Personal Belongings
The Student Union staff is not responsible for any personal property brought to the
facility. Please encourage your teen to keep their valuables at home.
Movies
The City of San Dimas Student Union provides its members with an opportunity to view movies
during various programs. Due to some of the movies’ content the ratings may range from PG to
PG-13. The movies viewed may include mature themes, strong language, mild violence or
comedic material. Staff will attempt to avoid any movies that contain inappropriate material,
such as excessive profane language, graphic violence and explicit sexual content.
Television
Teen members may also be given the opportunity to view television programs on local stations
or basic cable channels, but not premium or pay-per-view channels. Staff will try to avoid the
viewing of inappropriate programming when possible.
Video Games
Student Union members may also be given the opportunity to play video games with a rating of
T for Teens or lower.
Phone
Phone use is made available to teens only to get in contact with their parent or guardian.
Participants are always to ask a Staff member for permission before using the Student Union
phone (unless the call being made is for an emergency purpose.) Participants using the phone will
be required to indicate their parent or guardians phone number to Staff. The Student Union
phone number is (909) 394-6288. The following phone usage policies apply:
• No incoming calls to participants unless it’s a parent or guardian
• Calls are not to exceed 5 minutes unless it is an actual emergency
• Participants are not allowed to answer a ringing phone.
Phone use is a privilege and should not be abused. Failure to abide by these rules could lead to
suspension, permanent suspension or removal from the Student Union.
Snack Bar
The Student Union offers a snack bar which includes a variety of snacks for sale that your teen
participant can purchase during designated times while at The Student Union. A Student Union
Membership is required to access the Student Union Snack Bar.
Teen Action Committee (TAC) and Volunteering
Teen Volunteers must fill out a Teen Action Committee (TAC) application. A mini interview may
be necessary to better meet the needs of the volunteers for the center. We do not provide
volunteer opportunities for hours due to court ordered community service.
Participant Code of Conduct
The Student Union program is a quality experience for participants. We reserve the right to
ask any participant to leave the program for his or her own betterment or the welfare of the
group. The Student Union and its activities are a PRIVILEGE and the Participant Code of
Conduct must be observed by all participants. Rules and regulations are as follows:
Dress Code
Follow the same dress code rules that are expected at school (no gang attire, no mid drift
showing, etc.) (See attached)
Bullying, Harassment, Sexual/Hate Crimes
Bullying and Harassment of any kind will not be tolerated. The San Dimas Sheriff’s Department
will be notified and all applicable state and federal laws will be enforced.
Abusive Behavior
Abusive behavior is strictly prohibited. Abusive behavior is defined as:
A. Physical abuse - That which does harm to another's person or possession (i.e. hitting,
biting, kicking, pushing, spitting), includes also victimless and disruptive behavior.
B. Verbal abuse - that which is offensive and/or degrading to another individual (i.e. name
calling, swearing).
Illegal Substance, Weapons Drugs and Alcohol
Use of or possession of any illegal substance at the Student Union, or at any city-sponsored
activity is grounds for Police notification and permanent suspension.
Swearing, Profanity
Profanity, offensive language, obscene gestures or disorderly conduct is prohibited.
Additional Rules and Regulations
• Participant is responsible for his/her own property
• Food and drinks are allowed in designated areas only
• Equipment must be used properly
• Participants must keep their hands and feet to themselves
• Defacing facility or equipment is prohibited*
*The City of San Dimas reserves the right to invoice parents/guardians for
restitution/repair for damage/repairs of City property to include materials and labor.
• Respect the personal property of others
• Participants must follow instructions and direction of San Dimas Recreation Center
Staff
• No skateboarding, scooter or bike riding is allowed in or in front of the center
• No loitering before, during and/or after the program opens/closes
• Stealing or accepting any stolen property is strictly prohibited
• The Student Union policy is one of “hands-off.” Public Displays of Affection, which
may include kissing, handholding, or inappropriate hugging, are unacceptable and not
allowed.
Discipline
Staff will provide and maintain clear, reasonable limits for children's behavior. Positive
behaviors will be reinforced and negative behaviors identified and redirected.
Participants will be helped to recognize and identify their feelings as valid and acceptable.
Staff members will intercede if a child's behavior is harmful to him/herself or others. Staff
will plan ahead to try and prevent problems.
Your child(ren) will be free from corporal or unusual punishment, infliction of pain, humiliation,
intimidation, ridicule, coercion, threat, mental abuse, or other actions of a punitive nature.
Failure to comply with program rules and regulations may result in dismissal from the
program/facility for the day or possible dismissal for the remainder of the school year.

Student Union Liability Waiver (must be signed by parent/guardian):
I give permission for the minor in my custody to participate in the above-mentioned activities, and
I hereby release and discharge in advance the City of San Dimas, its officers, agents, employees
or any volunteers who may assist in said direction from and against any and all liability arising out
of, or connected in any way with said minor’s participation in said activity. I fully understand that
my participation, or that of the minor in my custody as registered, in the above-mentioned
activity, presents exposure to the risk of personal injury, death or property damage. I hereby
acknowledge that participation in this event/class is voluntary and agree to assume any such risks.
I hereby release, discharge and agree not to sue the City of San Dimas, its officers, agents,
employees or volunteers for any injury, death or damage to or loss of personal property arising out
of, or in connection with, participation in the event/class from whatever cause, including the active
or passive negligence of the City of San Dimas, its officers, agents, employees or volunteers or
