• Klein deel wordt chronisch • 10 tot 40 % • Belangrijke kost: werkverlet, medische kosten

Evidence Based Guidelines
European Guideline Acute LBP
• Summary of recommendations for diagnosis of acute non-specific low back pain:
• Case history and brief examination should be carried out • If history taking indicates possible serious spinal pathology or nerve root • syndrome, carry out more extensive physical examination including
neurological • screening when appropriate • Undertake diagnostic triage at the first assessment as basis for management • decisions • Be aware of psychosocial factors, and review them in detail if there is no • improvement • Diagnostic imaging tests (including X-rays, CT and MRI) are not routinely • indicated for non-specific low back pain • Reassess those patients who are not resolving within a few weeks after the first • visit, or those who are following a worsening course
European Guideline Acute LBP
• Summary of recommendations for treatment of acute non-specific low back pain:
• Give adequate information and reassure the patient • Do not prescribe bed rest as a treatment • Advise patients to stay active and continue normal daily activities
including work if possible • Prescribe medication, if necessary for pain relief; preferably to be
taken at regular intervals; first choice paracetamol, second choice NSAIDs
• Consider adding a short course of muscle relaxants on its own or added to NSAIDs, if paracetamol or NSAIDs have failed to reduce pain
• Consider (referral for) spinal manipulation for patients who are failing to return to normal activities
• Multidisciplinary treatment programmes in occupational settings may be an option for workers with sub-acute low back pain and sick leave for more than 4 - 8 weeks
European Guideline Chronic LBP
• Summary of the concepts of diagnosis in chronic low back pain (CLBP) • Patient assessment • Physical examination and case history:
The use of diagnostic triage, to exclude specific spinal pathology and nerve root pain, and the assessment of prognostic factors (yellow flags) are recommended. We cannot recommend spinal palpatory tests, soft tissue tests and segmental range of motion or straight leg raising tests (Lasegue) in the diagnosis of non- specific CLBP. Imaging: We do not recommend radiographic imaging (plain radiography, CT or MRI), bone scanning, SPECT, discography or facet nerve blocks for the diagnosis of non-specific CLBP unless a specific cause is strongly suspected. MRI is the best imaging procedure for use in diagnosing patients with radicular symptoms, or for those in whom discitis or neoplasm is suspected. Plain radiography is recommended for the assessment of structural deformities. Electromyography: We cannot recommend electromyography for the diagnosis of non-specific CLBP .
• Prognostic factors We recommend the assessment of work related factors, psychosocial distress, depressive mood, severity of pain and functional impact, prior episodes of LBP, extreme symptom reporting and patient expectations in the assessment of patients with non-specific CLBP.
European Guideline Chronic LBP
• Summary of the concepts of treatment of chronic low back pain (CLBP) • Conservative treatments: • Cognitive behavioural therapy, supervised exercise therapy, brief educational interventions, and
multidisciplinary (bio-psycho-social) treatment can each be recommended for non-specific CLBP. Back schools (for short-term improvement), and short courses of manipulation/mobilisation can also be considered. The use of physical therapies (heat/cold, traction, laser, ultrasound, short wave, interferential, massage, corsets) cannot be recommended. We do not recommend TENS.
• Pharmacological treatments: The short term use of NSAIDs and weak opioids can be recommended for pain relief. Noradrenergic or noradrenergic- serotoninergic antidepressants, muscle relaxants and capsicum plasters can be considered for pain relief. We cannot recommend the use of Gabapentin.
• Invasive treatments: Acupuncture, epidural corticosteroids, intra-articular (facet) steroid injections, local facet nerve blocks, trigger point injections, botulinum toxin, radiofrequency facet denervation, intradiscal radiofrequency lesioning, intradiscal electrothermal therapy, radiofrequency lesioning of the dorsal root ganglion, and spinal cord stimulation cannot be recommended for non-specific CLBP. Intradiscal injections and prolotherapy are not recommended. Percutaneous electrical nerve stimulation (PENS) and neuroreflexotherapy can be considered where available. Surgery for non-specific CLBP cannot be recommended unless 2 years of all other recommended conservative treatments — including multidisciplinary approaches with combined programs of cognitive intervention and exercises — have failed, or such combined programs are not available, and only then in carefully selected patients with maximum 2-level degenerative disc disease.

• Yellow flags: • A negative attitude that back pain is harmful or potentially severely
disabling • Fear avoidance behaviour and reduced activity levels • An expectation that passive, rather than active, treatment will be
beneficial • A tendency to depression, low morale, and social withdrawal • Social or financial problems

Conditie
Inhoud revalidatieprogramma
• Testing functionele weerslag, depressie, bewegingsvrees (vragenlijsten) bij begin en einde van de behandeling
• Informeren van de patiënt over de mechanische factoren die de aantasting van het wervelzuilsegment bevorderen (anatomie,biomechanica, wervelzuilbelasting), de uitleg over de grondslagen van de rugondersteuning en de bespreking van de emotionele en psychologische factoren die de pijn kunnen onderhouden;
• De praktische vorming in rugsparende technieken (wervelzuilontlasting) met inbegrip van frequente beroepsbezigheden (tillen van zware lasten,enz.);
• Evaluatie werkpost (indien van toepassing) en informatie over houding en aanpassing werkomgeving
• Een geïndividualiseerde revalidatie met tenminste een houdingscorrectie,een analytische versteviging van de paravertebrale spieren, en rekoefeningen;
