FULL WEEK COST -‐ FOR 3-‐5 YEARS = $1,000 / 6-‐12 YEARS = $1,300 DAY COST -‐ FOR 3-‐5 YEARS = $300 / 6-‐12 YEARS = $400
DECLARATIONS I agree to allow the participant to participate in HK Football Academy’s programme and confirm that the information given above is true and correct. If my application is successful, I shall be fully responsible for the payment of all the expenses and the officers, employees, servants, agents or licensees, shall not be liable or responsible to the participants or applicants for any loss, injury, damages, claims, cost or expenses whatsoever, which may be sustained by the participant and/or the lesson companion/helper, caused by or in consequence of HK Football Academy’s programme.
RULES & REGULATIONS All fees are non-‐refundable unless the student id not accepted into the class No refund or credit will be provided for students withdrawing after a course commences Training will be cancelled during thunder, red and black storm warnings, or the hoisting of the number 8 typhoon signal or inclement weather conditions. Lessons cancelled due to the adverse weather with not be refunded or rescheduled, a credit not will not be provided The Hong Kong Football Academy Ltd. Reserves the right to change course venue and time without prior notice. Please ensure to pick up your child promptly after the completion of every lesson
