Policy for Fall Prevention

Chip Eng Seng Contractors (1988) Pte Ltd is committed in providing a safe work environment for itsemployees to prevent occupational injuries due to falls.

Fall Prevention Plan is an integral part of our commitment to a safe working environment. At any timewhen worker is exposed to fall from height hazards, we will implement control measures fromengineering controls to PPE to reduce the risk level.

Fall Prevention is achieved through hierarchy controls that will involve all levels of management.Supervisors and workers are expected to access the risks associated with the task and ensure thatproper mitigation is in place to protect them while climbing and working at height. Where worker isunsure of the methods, equipment or procedures to reduce the risk they are to seek direction fromtheir supervisors

The application of this policy shall be outlined within the fall prevention plan and shall be responsibilityof every worker within the company. This policy is supported by the highest level of management andshall be enforced without exception. CES is committed to reduce and ultimately eliminate anyaccident/incident resulting from working at height.

ResponsibilitiesMr. Swee Ken peng (WSHO) is appointed as competent person to develop, implement, maintain and evaluate the Fall Prevention Plan.

Ensure that there is proper allocation of manpower and resources to ensure that the procedure is implemented at all site and reviewing the FPP quarterly with EHS department to ensure its effectiveness.

ResponsibilitiesWork at Height Supervisor

1.To ensure that the person working at height in a workplace only after he has received adequate safety and health training to familiarize himself with the hazards associated with work at height and the precautions to be observed.2.To inspect and ensure any travel restraint system or any fall arrest system to be used in a workplace to carry out at the workplace any work at height are in good working conditions and are safe to use.3.To inspect and ensure the anchorage and anchorage line of the travel restraint system or fall arrest system at the start of every work shift to ensure that they are in good working conditions and are safe for use. 4.To immediately remove from service and such anchorage or anchorage line which is found to be defective.5.To immediately enters the results of every inspections referred to in sub-paragraph (2 & 3 ) into a register containing such details as may be required by the Commissioner before the end of the work shift.6.To ensure no hazardous work at height without permit to work.7.To ensure application for a permit-to work to carry out any hazardous work at height shall be made by the supervisor of the person who is to carry out the hazardous work at height.8.Briefs / trains workers on the safe work procedures, risk assessment, in-house rules and regulations, and othersafety requirements in Tool Box Meetings or other appropriate training means, as applicable, and maintain such training records.9.To ensure PPE and fall prevention equipment are adequate and in good conditions (eg. Safety helmet, safety boots, safety belt / harness and hand gloves, etc).10.To ensure safe means of access or egress to the work area are provided

11.To ensure all persons subjected to falling risks are equipped with Personal Fall Arrest System (PFAS).12.To ensure edge protection provided when there is falling risk.13.To inspect anchorage and lifeline installed.14.To ensure travel restraint system used to exclude persons from falling risk.15.To ensure material, equipment and work tools are placed at least 1m away from edges of building.16.To ensure materials, equipment and work tools are checked and free of damage or defect.17.To ensure adequate lightings are provided along the access and the work area.18.To ensure adequate and secured life line with padding against wear and tear are provided.19.To ensure barricade are erected to prevent fall from height.20.To ensure work area in good working conditions.21.To ensure safety measures are implemented before apply the Permit To work.22.To ensure personnel are adequately trained to perform work at height.23.As a Risk Assessment team member to supports the WSH Officer in the identification of measures in the safe work procedures / practices and risk assessment report for works intended for execution24.As an Emergency Rescue Team member shall assist ERT Warden and Emergency Coordinator during an emergency rescue operation.25.As a member of site organization (refer to the Organization chart) you have to follow the instruction from the Project Director , Project Manager and to guide the supporting staffs

ResponsibilitiesWork at Height Assessor

1.Assess whether all reasonably practicable measures have been taken to ensure the safety and health of the person who will be carrying out the hazardous work at height; and2.inspect the site (including its surroundings) where the hazardous work at height is to be carried out together with the supervisor of the person who is to carry out the work, to ensure that the hazardous work at height can be carried out with due regard to the safety and health of the person and any other person at work in the factory who may be affected.3.If the work-at-height safety assessor is satisfied that the hazardous work at height can be carried out in the factory with due regard to the safety and health of persons who are to carry out the hazardous work at height and of other persons at work in the factory who may be affected, he shall endorse the application for the permit-to-work and forward the endorsed application to the authorised manager.4.It shall be the duty of the work-at-height safety assessor to exercise all due diligence when performing his functions in relation to the evaluation and endorsement of an application for a permit-to-work under paragraphs (1) and (2).

ResponsibilitiesWork at Height Authorised Manager

The authorized manager for any hazardous work at height at a factory may issue a permit-to-work in relation to any hazardous work at height which is to be carried out in the factory if the authorised manager is satisfied that

1.there has been a proper evaluation of the risks and hazards involved in the carrying out of the work based on the available information;2.no incompatible work which may pose a risk to the safety and health of other persons at work in the factory will be carried out at the same time in the same vicinity as the hazardous work at height;3.all reasonably practicable measures will or have been taken to ensure the safety and health of the persons who carry out or are to carry out the hazardous work at height; and4.all persons who are to carry out the hazardous work at height are informed of the hazards associated with it.5.An authorised manager who issues a permit-to-work in respect of any hazardous work at height shall retain a copy of the permit-to-work.6.It shall be the duty of the authorised manager to exercise all due diligence when performing his function in relation to the issuance of a permit-to-work under paragraph (1).7.Subject to regulation 28, a permit-to-work shall be valid for the period stated therein, and if the hazardous work at height for which the permit-to-work is issued is not completed within the validity period, a fresh application shall be made in accordance with regulation 22.

Platform were installed onto the precast wall at the ground

level before hoisting to eliminate the risk of scaffold

erectors exposed to falling from the edge of the building or

any open sides this is to provide safe access for tower

crane erector for safe access during tie back installation

Incident/Accident Investigation and Analysis

All Incidents/Accidents that resulted in injuries to workers, as well as near misses, regardless of the nature, shall be

reported to immediate supervisor who shall inform the WSHO and Management immediately.

•Investigations shall be conducted to determine any underlying deficiencies in the existing underlying Fall

Preventation Plan which may cause or contribute to the occurrence of the incident;

•In event of such as accident/incident, the fall preventation plan shall be re-evaluated to determine whether that

additional measures ( Practices, Procedures, Training) are necessary to prevent recurrence

•Identify the opportunities for preventive measures to be implemented to prevent any incident form

•The results from any such investigated (Inclusive of lesson learnt and the corrective and preventive actions to be

taken)should be promptly communicated to all relevant parties.

•A record of all accidents and incidents and their investigation findings and outcomes should be properly

documented.
