Disease Of The Month Retinal Detachment

1

Overview: Retinal detachment describes an

emergency situation in which a thin layer of tissue

(the retina) at the back of the eye pulls away from its

normal position. A retinal detachment separates the

retinal cells from the layer of blood vessels that

provides oxygen and nourishment. The longer retinal

detachment goes untreated, the greater your risk of

permanent vision loss in the affected eye.

Background: The retina is the light-sensitive tissue

lining the back of our eye. The retina converts the

light rays into impulses that travel through the optic

nerve to our brain, where they are interpreted as the

images we see. A healthy, intact retina is key to clear

vision.

Causes: They can be caused by a hole or tear in the

retina that allows fluid to pass through and collect

underneath the retina, detaching it from its

underlying blood supply. Retinal tears can develop

when the vitreous gel separates from the retina as

part of aging or in patients with abnormal thinning in

the peripheral retina or occasionally from trauma. Tractional retinal detachments are caused by scar

tissue that grows on the surface of the retina and

pulls the retina off the back wall of the eye. This type

of retinal detachment may occur from diabetes or

2

other conditions. Other retinal detachments form

when fluid leaks out of blood vessels and

accumulates under the retina.

Signs and Symptoms: A sudden increase in size

and number of floaters, sudden appearance

of flashes, having a shadow appear in the periphery

(side) of your field of vision, seeing a gray curtain

moving across your field of vision, or a sudden

decrease in vision. Flashes and floaters in

themselves are quite common and do not always

mean you have a retinal tear or detachment.

Treatment: Detachments that involve the center of

our vision (macula) need to undergo surgery within

7-10 days and if the macula has yet to be involved

surgical intervention needs to be performed urgently

to save central vision. Surgery involves laser

treatments, cryotherapies, and scleral buckles to

reattach the retina and to prevent further

detachment.

Follow up: 1 day, 1 week, 2 weeks, 1 month, 3

months, 6 months

Optical

Anti-glare X Perio UV:

• This is an anti glare treatment that is for a sunglass lens

for frontside and backside elimination of glare. This

treatment also polarizes the lens, which eliminates all

horizontal glare. We recommend this treatment to all

patients to avoid eyestrain during all weather conditions.

This product also provides UV protection. The doctors

recommend this product and we place a 1-year warranty

on the lenses.

Contact Lens

Multi Focal Soft Contact Lenses:

• Contact lenses that contain two or more prescriptions

(or ‘powers’) mainly used to correct distance as well as

near. Multifocal contacts are used primarily for our

presbyopic patients who require a distance and near

prescription although they can be used in younger

populations who experience strain at near. These

lenses are a great option for individuals who don’t like

the idea of having to wear reading glasses or are

current or past contact lens wearers. The two most

common designs of these lenses are concentric and

aspheric (see margin figures). These contacts have the

same comfort as distance contacts and require the

same cleaning and disinfecting practices as other soft

lenses. We always recommend a single use lens but

they also come in two week and monthly options.

Thank you all for your understanding and patience during these

unprecedented times. COVID 19 has interrupted everyone’s lives

in many ways. As essential healthcare providers it is our

responsibility to provide our community access to medical eye

care to protect our patients from potentially blinding conditions

as well as easing the burden of our hospital workers. We will

continue to work together to provide and a clean, sanitary clinic

as well as provide the same quality care our patients expect.

Please stay safe during these trying times and we look forward to

when normalcy can return.
