The Global Burden of HCV Infection
• Approximately 170 million people are chronically infected with HCV
• HCV is responsible for >350,000 deaths per year worldwide- 60,000 in Europe- >7,500 in the US
• Up to 85% of HCV-infected patients are unaware of their status- 50.3% in the US- 42.6% in France (i.e., 100,000 persons)
• Less than 10% of the HCV-infected population has received treatment in the US

• Sensibilité – 3 fmol/L (i.e ~ 500 to 3 000 UI/mL d’ARN VHC)– Tous les génotypes (excepté génotype 2)
• Intervalle de quantification– 3 à 20 000 fmol/L (≤ 7,8 Log10 UI/mL d’ARN VHC)
• Stable à 37°C pendant 96 heures

- Identify active viral replication
- Make appropriate decisions. Shorten or extend treatment duration
. Stop treatment due to futility
Identify treatment failure and the emergence of resistance. Virological failure is associated with selection of viral
variants with reduced susceptibility to DAAs

Requirements for HCV RNA Assays
• Real-time-based assays with– A low lower limit of detection– A broader range of linear quantification– Identical lower limits of detection (LLOD) and
quantification (LLOQ)
• Satisfactory analytical performance, regardless of the HCV genotype

HCV RNA Level Assessment in the Era of DAA
• HCV RNA quantification should be regularly assessed before, during and after treatment
• HCV RNA quantification should be based on a real-time PCR (or TMA) assay– With results expressed in IU/mL– With identical LLOD and LLOQ, of the order of 10-15 IU/mL
• In a given patient, HCV RNA level monitoring must be performed with the same real-time assay

Europe or FDA Approval for HCV RNA Quantification Tests
• Only 2 tests are approved by the FDA and in Europe for HCV RNA quantification in clinical practice– m2000 since May 2011– New version of CAP/CTM since March 2013

longitudinales
