Our Ref: OHS-UT/CAPT/CIR/CHECKOUTPROCEDURE/20131113.DOC 14 November 2013 CHECK-OUT PROCEDURE With the impending check-out at the end of the semester, please take note of the proper check-out procedure below: 1. CHECK-OUT DATE a) Official last day of accommodation for Semester 1 AY13/14 is 9 December 2013, before 12:00noon for residents without vacation stay. b) Check-out can only be done during office hours. c) Check-out and returning of key(s) must be done personally. d) For residents on partial Vacation Stay, you are required to check out on the end date that you have indicated and paid during your application for Vacation Stay. 2. ROOM INSPECTIONS a) All residents must make an appointment with the Management Office for a room inspection. b) Room inspection must be done at least 3 working days before your intended check-out date. c) All room inspections must be carried out during office hours (from 9.00am to 5.00pm). d) Prior to the room inspection, please ensure the room is tidied and cleaned to the condition as shown in the attached picture and all inventories are in order during the inspection. e) Resident must be present during the room inspection. f) During the room inspection, resident will be asked to fill up the room and EVS (air-con) check-out form. g) You will be requested to sign on the inspection check list after the completion of the room inspection. 3. BEFORE YOU PROCEED TO THE CHECK-OUT COUNTER a) Before your check-out day, please ensure that you use-up your EVS and meal plan credits. You may log-in to the following website to check your credit balance: https://aces01.nus.edu.sg/Prjhml/login.do (Meal plan credits) https://nus-utown.evs.com.sg/ (EVS credits for air-con room only) b) On your check-out day, please ensure the following is completed: i. Switch off all lights, fans and air-conditioner (if applicable) in your room/apartment ii. Ensure that all windows are closed iii. Clear all personal belongings from the room and living room (in Apartment) iv. Clean the room and the common areas of the apartment (where applicable) to the conditions as shown in the attached picture v. Lock the room and apartment door when you leave to proceed to the check-out counter c) Kindly note that any belongings left behind in the room / common areas of the apartment after check-out will be discarded and the resident will be liable for any additional housekeeping fee. As such, please ensure that you take all your personal belongings with you and discard off any unwanted items properly. 4. AT THE CHECK-OUT COUNTER a) On your check-out day, proceed to the Management Office / Check-Out counter personally to return the following items: i. Access Card (for residents who are still holding on to the UTown access card) ii. Mailbox Key iii. Mobile pedestal Key(s) iv. Wardrobe Drawer Key(s) v. Fire Warden Box Key (only applicable to appointed Fire Wardens) b) Settle in full any outstanding fees (accommodation / meal plan / loss of access cards / reprogramming fee loss of any keys). CHECK-OUT PROCEDURE With the impending check-out at the end of the semester, please take note of the proper check-out procedure below:

1. CHECK-OUT DATE
